All bids and all attachments must be bound and delivered sealed to the county at the address shown below no later than the time and date set for receipt of bids. The front lower left corner of each sealed envelope/package shall contain the following information for proper identification: wakulla gardens phase iii construction itb # 2022-31 attention: patty taylor 1. Include name and address of bidder on each sealed envelope/package. 2. If bid is contained in multiple packages, number each sealed package sequentially, I.E., "1 of 3", "2 of 3", "3 of 3". Bidder shall construct its bid in the following format. Required forms can also be provided in microsoft word format, upon request. Proposal transmittal form on the firms letterhead (bid proposal form 1) all signatures must be by an individual with authority to legally bind the bidder, witnessed, and corporate and/or notary seal (as applicable.) if the individual signing the proposal transmittal form does not have apparent authority to legally bind the bidder, attach documentation demonstrating such authority. The corporate or mailing address must match the company information as it is listed on the florida department of state division of corporations. Attach a copy of the webpage(s) from as certification of this required information. Verify that all addenda and tax identification number have been provided. Bid proposal, basis of bid and bid bond (bid proposal form 2) the basis of bid form is included with the bidding documents. Additional copies may be obtained from the procurement office. All blanks on the bid form shall be completed in ink and the bid signed in ink. Erasures or alterations shall be initialed in ink by the person signing the bid from. A bid price shall be indicated for each bid item, and unit price item listed therein, or the words "No bid," "No change," or "Not applicable" entered. In the event of multiplication/addition error(s), the unit price shall prevail. Prices written in words shall prevail over figures where applicable. All bids will be reviewed mathematically and corrected, if necessary, using these standards, prior to further evaluation. A bid bond is required for 5% of the amount of the bid. The county does not have a specific bond form bidder must use, but the bond must be underwritten by a surety as described in section 4 of the draft construction agreement. (intent and general information, appendix b). Required forms, documents and certifications (bid forms 3 through 5) the following forms must be fully filled out and signed by a person with authority to bind the bidder: bid proposal form 3: list of proposed subcontractors bid proposal form 4: comments on proposed contract bid proposal form 5: qualification questionnaire This project is located in Wakulla County, Florida, and includes the third phase of construction of a gravity sewer system servicing the residential area known as Wakulla Gardens. Wakulla Garden Phase III is located on the east side of Spring Creek Road, roughly a half mile north of Dr. M.L.K. Jr Memorial Rd. This phase will include construction of a sanitary lift station, approximately 15,350 LF of 8", 10 LF of 10", and 183 LF of 15" gravity sewer, 3,250 LF of 6" force main, and service to approximately 224 existing homes and 206 vacant parcels. The Phase III 6" force main will connect to a gravity manhole installed as part of this project, and discharge into the Wakulla Gardens Master Lift Station at the intersection of Spring Creek Rd. and Dr. MLK Jr. Memorial Road. Also included in this project is a series of two manholes at the Master Lift Station that provides provisions for future phases connections. At two locations where gravity sewer was not feasible, grinder pump station stub-outs will be provided, the low pressure lines will be installed and complete for customer connection at a later date. Work at existing homes will be a complete plumbing connection and demolition of existing septic tanks per Health Department rules. Restoration work will include paving impacted roads per FDOT Specifications, drainage improvements, and complete restoration of impacted areas within the project site. Trench excavation, including necessary pavement removal, rock removal, and muck removal. Sheeting and shoring Clearing, grubbing and grading. Erosion Control Excavation, including necessary pavement removal, rock removal, muck removal, and existing sewer removal. Dewatering and disposal of surplus water. Structural fill, backfill and grading. Placing system in operation. Any material and equipment required to be installed and utilized for tests. Maintaining the existing quality of service during construction. Appurtenant work as required for a complete and operable system. Operation and Maintenance manuals. Bypass pumping. Maintenance of Traffic.
