A description of the Project is as follows:Description of the Services: Services include but are not limited to furnishing of all labor, materials, equipment, services and incidentals for testing and potential repair work to deep injection wells and or associated monitored wells. Performance of Mechanical Integrity Test (MIT) of Class I injection wells and ancillary work necessary for the MITs is included in the Agreement. Repair work identified at a future date as needed for existing deep wells and monitor wells is also included. All work is to be performed on wells owned and operated by the City. MIT schedules are identified in the bid document. Maintenance of operations is mandated throughout the testing period and all work, including scheduling shut-downs, shall be coordinated with and approved by the City.Qualification Requirements: 1. Contractor shall be a Florida Licensed Water Well Contractor and license documents from the South Florida Water Management District shall be provided with the bid. 2. The contractor shall provide a list of recent projects (ten projects minimum), in which mechanical integrity testing (MIT) and deep well repair work has been successfully completed by the contractor and references can be verified. 3. The contractor shall have at least three years of consecutive experience performing deep well MIT and repairs work in the State of Florida. The City shall be the sole and final judge in determining if the Contractor meets the Qualification Requirement. It will be the responsibility of the bidder to insure that bids are received no later than the date and time specified above. Bids received after that time will not be considered.This bid may be obtained at electronically.
