The purpose of this project is to renovate portions of Building 1 and Building 25 for Lab, Compensation & Pension, and Radiology on the NWIHCS Omaha VA Medical Center campus. Building 25 is a single-story addition, contiguous with the main hospital building, that was constructed in the mid-1980s as an Outpatient Clinic facility. This project will renovate approximately 16,250 square feet of existing space on the first floor of Building 1 (6,900 square feet for Outpatient Lab and Compensation and Pension) and Building 25 (9,350 square feet for Radiology). The work covered by this Statement of Work (SOW) consists of furnishing all design, labor, equipment, materials, travel, testing, and permits required to perform the task described herein within the framework of the contract. The work will be required to be completed within 480 calendar days from Notice to Proceed (NTP). 1.2. Location of Project: 4101 Woolworth Avenue, Omaha, NE 68105. See plans for details. 1.3. SIZE SQ FT OF NEW/RENO: Approximately 16,250 square feet RENO 1.4. Contractor shall remove all debris from VA property, and provide waste reports to Contracting Officer Representative (COR). 1.5. Prime Contractor Superintendent must be on site while work is being performed and must be OSHA-30 certified. 1.6. Prior to work commencing, Contractor must check-in with COR, in room B618. 1.7. Dust created during construction must be contained and prevented from leaving the work area. 1.8. Contractor must maintain a clean working site. 1.9. All work must be contained within the approved construction limits. 1.10. Contractor shall follow all security protocols. 1.11. Contractor shall notify COR at least 48 hours prior to any significant construction activity.
