MBE/FBE Subcontracting and Supplier Goals Apply. Project Labor Agreement applies. The BEP goals established for this project are: General Trade: 20% MBE, 5% WBE, 2% PBE/VBE Combined Plumbing Trade: 20% MBE, 5% WBE, 1% PBE/VBE Combined Ventilating Trade: 10% MBE, 5% WBE, NO PBE/VBE Goal Electrical Trade: 20% MBE, 5% WBE, 1% PBE/VBE Goal Combined When completing the 4105 Form, use full complete name, address and phone number for each MBE, WBE, PBE, and VBE certified firm on the 4105 Form. Do not use acronyms unless that is the name of record for the firm. List the dollar amount to be paid to each firm or the dollar amount to be performed through own forces, not the percentage. Indicate the firm's designation. A firm can only be designated as one, i.e. MBE or WBE, not both. List the specific commodity code(s) that apply to the work to be done and be sure the firm is certified in each commodity code. Verify that the goals on this project are met for each specific goal. If there are any bid revisions, remember to revise the 4105 Form accordingly
