Abatement of Asbestos-containing Floor Tile and Mastic approximately 15,525 feet. The Town of Braintree, Braintree Public Schools invites sealed bids for the abatement of approximately 12,000 square feet of asbestos-containing floor tile and mastic at the Flaherty Elementary School, 99 Lakeside Drive, Braintree, MA 02184, pursuant to M.G.L. c.149 44A-H. Furnish material and labor to remove, contain and properly dispose of the existing VAT flooring and mastic from specified areas in the Flaherty Elementary School, 99 Lakeside Drive, Braintree, MA 02184. B. Acquire all necessary permits from the Town of Braintree and the Commonwealth of Massachusetts. C. Abate all the asbestos containing material (ACM) and VAT material and provide phase contrast microscopy (PCM) testing by a Massachusetts certified abatement company. Work includes removal of all floor tile, adhesive material, ACM and proper off-site dispose at a license facility in accordance with federal, state and local regulations. Asbestos abatement and air testing work and procedures concerning abatement will be performed by a Massachusetts licensed asbestos abatement contractor. Removal of ACM shall be subject to "third party" performance of air quality and visual inspection to ensure that the work will meet or exceed federal, state and local regulations that govern removal of ACM.. All work shall be done in accordance with applicable regulations. D. Communicate with the state Department of Environmental Protection (DEP) prior to the start of work to submit any required notifications or applications, provide them with the project start date, estimated quantities of materials, licensure information and any other information that may be required. Follow applicable procedures and await DEP authorization prior to beginning work. E. Demolition work shall also include the old cove base (if any). F. Off-site legal disposal of all asbestos floor tile and associated materials and coverings is the responsibility of the prime contractor. Supply all secure dumpsters and secure containers required for the safe removal of material from the site. The prime contractor shall provide proof that the disposal site(s) for hazardous waste and non-hazardous waste materials have valid and current permits to dispose of waste. The Town will remove all furniture and all non- contaminated movable objects from the work area. The Town will supply ample water and electrical service for the project. G. Receipts shall be obtained by the prime contractor from the disposal site(s) and submitted to the Awarding Authority upon request for final payment. As a condition of final approval and acceptance, there shall be two affidavits submitted, one in support of a manifest covering all ACM and a second advising that all other material from removal of the floor tile has been properly disposed of with an identification of the disposal site
