It is the Agency's intent to remove and replace the damaged concrete pad and repair a leak in a 24 inch pipeline located at the northwest corner on Haven Ave. and Banyan St. 1) Coordinate with Southern California Edison to de-energize power supply from pedestal and follow IEUA, MWD procedures (attached) to power down cabinets, Notify the church property owner two-week before groundbreaking. 2) Contact MWD Control Center for impending power-down no less than 72 hours prior to the event, 3) Salvage existing cabinets and their contents. Store in secure place, 4) Protect all conductors, wires, cables, and store in the nearest vaults, pull boxes when possible, 5) Demolish and remove damaged PCC pad, 6) Scrub and remove tree stump and roots in its entirety, 7) Unclassified excavation and grading with imports compacted to 95%, 8) Install 12' x 4' x 6.5" PCC pad and household boarders with minimum 2,500 psi Class 2 Portland Concrete Cement or equivalent with 316 Stainless Steel Hex head Wedge-bolt 4" deep min. Contractor is to field verify existing size and type of hardware needed for installation (Sheet P-06, E-02, E-05), 9) Re-install all cabinets and their contents. Connect all conductors, wires, and cables per MWD Special Instructions (Attached), 10) Excavate between IEUA Vault 1016280 to MWD Vault to expose an 18" Type 316 SS 3/8" wall pipe and a 24" CML&C steel 3/8" wall pipe joined by two flanges at approximate 7 ft below grade to top of the pipe (Sheet P-09), 11) Contractor is to locate the leak(s), replace the faulty component(s) with like-kind material 12) Perform hydrostatic test and backfill per as-builts, 13) Replace ground rod, 14) Restore landscapes and sprinkler system with like-kind materials, 15) Coordinate with SCE and/or City for permits and inspections when needed.
