SUNY Upstate Medical University (SUNY Upstate) intends to renew its Pathology Laboratory Proficiency Testing per New York State Department of Health (NYSDOH) pursuant to its discretionary authority Requirements: Must be able to provide 100% of all testing done at SUNY Upstate Downtown Campus Must be able to provide (3) references from like size teaching hospitals in the New York State that demonstrates the ability to provide Pathology Laboratory Proficiency Testing Must be CMS approved Must be NYSDOH approved per Subpart I (42 CFR 493 subpart I) Service-Disabled Veteran-Owned Set Aside: No Minority Owned Sub-Contracting Goal: 0% Women Owned Sub-Contracting Goal: 0% Service-Disabled Veteran-Owned Business Contracting Goal: 0% Disadvantaged Owned Sub-Contracting Goal: 0% Contract term: 2 Years
