L4379 Request for Proposals For Construction Services Hazard Mitigation Grant Program (HMGP) Generator Projects #1 RFP#: 2021-04 Federal Emergency Management Agency DR4399-FL Program Projects # 046-R, 060-R & 094-R (ADVERTISEMENT) You may direct any questions or schedule visits to the project site with Sherri Griffin. A. Evaluate electrical loads and properly size the generator system for each facility. Develop the necessary design plans to secure the necessary Building Permits to complete the installations.
B. The Finn/Contractor shall be responsible for furnishing or contracting all labor, materials, equipment, tools, transportation, and supervision and for performing all work C. The Firm/Contractor shall be responsible for maintaining a safe and secure worksite for the duration of the work. The Firm/Contractor shall maintain all work staging areas in a neat and presentable condition.
D. The Firm/Contractor shall ensure that no contractors or subcontractors are debarred or suspended from participating in federally funded projects. E. The Finn/Contractor shall have a current and valid occupational license/business tax receipt issued for the type of services being performed and shall provide a copy as part of entering into a contract with the Town. a. The Firm/Contractor shall provide copies of professional licenses for contractors selected to perform services. The Firm/Contractor shall provide a copy of a current and valid occupational license or business tax receipt issued for the type of services to be performed by the selected contractor. F. The Finn/Contractor shall provide an executed "Debarment, Suspension, Ineligibility, Voluntary Exclusion Form" for each contractor and/or subcontractor performing services under this agreement.
G. Secure ground disturbance permit / approval. Clear and grub the site for both the generator and fuel tank. a. The Finn/Contractor agrees to notify Town if any project funded under this Scope of Work will involve ground disturbing activities, including, but not limited to; subsurface disturbance; removal of trees; excavation of footings and foundations, and installation of utilities (such as water, sewer, storm drains, electrical, gas, leach lines and septic tanks) except where these activities are restricted solely to areas previously disturbed by the installation, replacement or maintenance of such utilities.
b. Successful Firm/Contractor shall provide, as part of the completion of each site, a letter/statement confirming that the actual ground disturbance area associated with the project work area(s).
