The Department of Facilities Management intends to purchase services for Preventative Maintenance Service and repair of all scissors lift, single man lift, boom lift, forklift and any other lift or material handling equipment at several locations, and will award to vendors who meet minimum qualifications. The intent of this invitation for bids is to identify and qualify contractors to provide factory recommended Preventative Maintenance Service and repair of all scissors lift, single man lift, boom lift, forklift and any other lift or material handling equipment at several locations for Maricopa County Facilities Management Department (FMD). All work is to be completed according to the manufacturer's specifications and per all local, State and Federal guidelines and follow Occupational Safety and Health Administration (OSHA), American National Standards Institute (ANSI). (See Line Items for Maricopa County's equipment inventory and locations). The PM activities will be completed twice yearly at six (6) month intervals and will be separate from the annual inspection and certification of the equipment to be conducted by a different vendor. Preventive Maintenance The preventive maintenance shall be in accordance with the manufacturer's recommendations based around applicable OSHA and ANSI standardsand shall be an all-inclusive price to include all parts and labor at 6-month intervals. * Semiannual Preventive Maintenance * Annual Preventative Maintenance Preventive Maintenance Summary Report to be sent with the invoice to show what work was done and what repairs may be needed and request approval from the County prior to work being completed. Approval shall be in writing via e-mail with the County. All repairs will be issued a separate work order and billed separate from the PM service. All service/repair work will be billed as time and material and will be separate from the preventative maintenance. Repairs will require a work order to be issued as an authorization for the contractor to proceed. Regular Work Hours 6:00 a.m. to 6:00 p.m. After Hours 6:00 p.m. to 6:00 a.m. Weekends 6:00 p.m. Friday to Monday 6:00 a.m. Holidays only as recognized as by the County All questions must be submitted through Electronically ONLY by the date and time listed in the bid. Q & A deadline: Nov 29, 2021 4:00:00 PM EST Contract duration: 1 year Contract renewal: 2 annual renewals Prices good for: 30 days Regions: Arizona, Maricopa Award is made at the sole discretion of the solicitation issuing Department. Maricopa County reserves the right to award in full or in part to a single vendor or multiple vendors.
