1. This solicitation is to establish a Master Agreement for "Barcode Labels for Highway Signs." Barcode labels must be single color black-type style, have a unique sequential nine digit alpha-numeric code, be two inch by one inch in size, and of material type UV intensified anodized aluminum. - The material thickness must be between 0.003 inches and 0.005 inches. - The text "KY Transportation Cabinet" shall be printed on each label. - The barcode value (human readable equivalent) shall be printed below the barcode. - Barcode labels must be pressure sensitive self-adhesive and bond to plastic surfaces, powder coated metal surfaces and slightly oily metal surfaces. - Defective barcode labels shall be replaced within thirty days. - Adhesive must have a shelf life of at least twelve months. - Minimum order shall be three thousand labels. - Vendor must track barcode value such that other entities besides KYTC can order barcode labels without duplication of barcode value. - Provide a Monthly report of barcode value range of labels delivered to the Kentucky Department of Highways Division of Maintenance. - Invoices are to be sent to the delivery location. See list below. - Price shall include delivery and lifetime warranty (guaranteed readable for product life).
