Proposals shall be made substantially in accordance with the accompanying Proposal Form and all blank spaces in the form for the trades on which the proposal is based shall be fully filled in; the signature shall be in longhand; the completed form shall be without interlineations, alteration or erasure. Two such copies will be provided Proposals shall be enclosed in an opaque sealed envelope, and shall bear the name of the project and the name of the bidder. Each Respondent submitting a proposal to the Jackson District Library for any portion of the work contemplated by the documents on which a response is based shall execute and attach the attached affidavit to the effect that it has not colluded with any other person, firm, or corporation in regard to any proposal submitted. Before executing any subcontract, the successful Respondent shall submit the name of any proposed subcontractor on the attached Subcontractor list for prior approval. Attention of Respondents is particularly called to the requirements for ensuring that employees and applicants for employment are not discriminated on any illegal basis per all Equal Employment Opportunity laws. The Attached Equal Employment Opportunity Certification shall be full executed and included with the Form of Proposal in the bid. Each bidder shall submit with his bid evidence that he is a certified installer of Jahn masonry repair products. Should a bidder find discrepancies in, or omissions from the drawings or specifications, or should he be in doubt as to their meaning, he should at once notify the Architects, who will send a written instruction to all bidders. Neither the Owner nor the Architect will be responsible for any oral instructions. The successful bidder shall be required to carry adequate Workmen's Compensation, public liability and property damage insurance. The Jackson District Library reserves the right to reject any or all bids, waive any irregularities and to award a contract, that in their sole opinion, is in the best interest of The Jackson District Library Mandatory alternates, if any, will be considered by The Jackson District Library in making an award. Voluntary alternates and/or substitutions which have not been approved by Addendum prior to the bid opening will NOT be considered in the selection of the successful bidder. Any bidder wishing to obtain Addendum approval of a voluntary alternate and/or substitution is referred to Article 4.4 of the General Conditions as supplemented. A tabulation of all bids received and the name of the successful bidder will be sent to all bidders.
