Amendment to Solicitation No. 75H71322Q00015, to incorporate photos of the generator and transfer switch within the Statement of Work. Amendment to Solicitation No. 75H71322Q00015 to include the date (Month, Year) for clause HHSAR 352.232-71 Electronic Submission of Payment Requests. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation no. 75H71322Q00015 has been issued as a Request For Quotation for a firm fixed price contract for a brand name Kohler 20Kw generator with transfer switch plus installation of the aforementioned generator, Salem, Oregon. See attached Statement of Work. The required delivery date is June 31, 2022. The delivery location and place of acceptance is Chemawa Indian Health Center, Western Oregon Service Unit, 3750 Chemawa Rd NE Salem, OR 97305, F.O.B. Destination. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-05 dated March 07, 2022. The provision at 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. The provision at 52.212-2, Evaluation - Commercial Items, applies to this acquisition. The following factors shall be used to evaluate offers: price and past performance. (End of Clause) This requirement is set-aside for TOTAL SMALL BUSINESS. The NAICS code is 238210 and the small business size standard is $15M. ATTACHMENTS: ATTACHMETN 1 - STATEMENT OF WORK ATTACHMENT 2 - WAGE DETERMINATION QUOTATION INSTRUCTIONS: The Government will consider competitive rates. Quoter shall submit quote. Hourly Rate/Price shall be an All-Inclusive Hourly Rate that includes travel, per diem, lodging/housing, transportation, meals, and applicable taxes (i.e. federal, state and local). The basis of award is the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Other factors for this solicitation include past performance of the quoter. Past performance information may be based on the Government's knowledge of and previous experience with the quoter, or other reasonable basis. Award will be made to the responsible respondent pursuant to FAR subpart 9.1. Government is only accepting inquiries/questions via e-mail. Provide and install the Kohler 20kW generator with transfer switch system and all its related equipment necessary to ensure system is reliable, contract shall provide warranty information and applicable user manuals. The goal of the Kohler 20Kw generator is to meet the needs of expanded services and power loads of the facility. This will be a replacement of the facility's existing Kohler generator and transfer switch in the same place where the old unit is currently located The contractor shall comply with all applicable Federal, State and local health regulations. The contractor shall accomplish the following services: 2.2.2. a The contractor shall replace and install the Kohler 20Kw generator and transfer switch in accordance with manufacturer's installation instructions 2.2.2. b. The contractor shall remove the smaller Kohler generator and transfer switch, and aforementioned installation of both new 20Kw generator and transfer switch in the same location. 2.2.2.c. The contractor shall install electrical and natural gas hookup. 2.2.2. d. The contractor shall train staff to switch the generator manually in the event Chemawa Indian Health facility undergoes transfer switch failure. 2.2.2. e. At the conclusion of the project, the contractor shall ensure proper operation of the Kohler 20kW generator with all system testing being witnessed by Government Point of Contact. 2.2.2. f.
