A fully completed Form HUD-5369-A, "Representations, Certifications and Other Statements of Bidders". Florida Contracting License. EPA, Lead Safety for Renovation Repair and Painting Certification. Small businesses and minority firms and section three businesses are urged to submit proposals. Certification as a Minority-business Enterprise (or number of partners, shareholders, employees who are members of minority classification or are women) and section three businesses should be included in the Bid proposal. Refer to Articles 38, 39 and 40 of The General Conditions. The PHA reserves the right to reject any and all bids, and to waive irregularities and formalities in the bidding. No bids may be withdrawn for a period of sixty days subsequent to the opening of bids without PHA consent.
