The District is requesting the submission of Pre-Qualification Applications from firms interested in providing General Contracting; and Mechanical, Electrical or Plumbing ("MEP") Services, to become eligible for the related work associated with the various Measure 'G' lease-leaseback projects. The District is a public school district located in Norwalk, California. The District has approximately 16,930 students serving the communities of Norwalk and La Mirada; including some surrounding cities, such as an unincorporated area of Los Angeles County. All submissions shall be sealed in a package plainly marked "Confidential Pre-Qualification Application." Please be advised, faxed and/or electronic submissions will not be accepted. All communications pertaining to the application will only be made through the Purchasing Department, Director (Ms. Cindy Jimenez) at bids@nlmusd.k12.ca.us. All Responses and other materials submitted in regards to this Pre-Qualification Application shall become the property of the District. All questions in the questionnaire must be answered. If a question is not applicable, then indicate a response of "N/A". "You" or "Yours" as used herein refers to the prospective bidders' firm and any of its owners, officers, principals and qualifying individuals. Any references to owners, officer, principals or partners herein shall include any qualifying individuals including any RME or RMO. Each questionnaire must be signed under penalty of perjury in the manner designated at the end of the form, by an individual who has the legal authority to bind the contractor on whose behalf that person is signing. If any information provided by a prospective bidder becomes inaccurate, the prospective bidder must immediately notify the District and provide updated accurate information in writing, under penalty of perjury. Each prospective bidder shall have a duly authorized owner, officer or principal complete the questionnaire and verify the truth of the information provided therein and in the financial statement.
