Project Location- The project has two base bids (A and B) and one alternate. Base Bid A includes removal and replacement of approximately 900 feet of sidewalk on the east side of Villa Park Drive, as indicated on page VP-1. Base Bid B and Alternate 1 include removal and replacement of sidewalk at various locations within the City.A list of locations for sidewalk removal and replacement, in association with the project alternate 1, are indicated on page L-1 through L-4. The list of locations is not all-inclusive, and locations may be added or deleted during the project. Notice of sidewalk repair has been provided to all property owners listed on pages L-1 through L-4. Property owners are allowed, by permit, to complete repairs prior to the Contractor commencing work. The Contractor will not be required to make repairs at locations where permits are issued to property owners. The Contractor will be notified of the permit locations prior to work in each area. This project consists primarily of the removal and replacement of deteriorated sidewalk, and the construction or replacement of curb ramps at various locations. Significant types of work and approximate quantities include: Clearing and Grubbing 1 LS Remove and Replace 4-Inch Concrete Sidewalk 15,700 SF Remove and Replace 6-Inch Concrete Sidewalk 2,650 SF Furnish and Place Topsoil, Seed, Fertilizer and Water 7,300 SY Contract 22-04 is a unit price contract. The estimated quantities as indicated above and in the contract Bid Proposal are inserted only for the purpose of determining the low bidder. No bid will be received unless accompanied by a Certified Check or a Bid Bond equal to 5% of the bid, payable to the City of West Bend, Wisconsin, as a guarantee that, if the bid is accepted, the successful bidder will execute and file the contract and payment and performance assurances, forming a part of the Specifications, within 10 days after the award of the contract. The payment and performance assurances must be for an amount at least equal to the contract price and must be in the form of a bond, an irrevocable letter of credit, or an escrow account acceptable to the City. In case the bidder fails to file such contract, performance bond, and payment bond, the amount of the check or bid bond shall be forfeited to the City as liquidated damages (Section 62.15(3) of Wisconsin Statutes).
