Contractor Awarded Address: Sumner, WA 98391-6290 USA Base and All Options Value (Total Contract Value): $71228 This opportunity is only available to small businesses. The U.S. Fish & Wildlife Service has a requirement to remove about 100 to 130 cubic yards of river sediment at the Pump House located at Makah National Fish Hatchery, in Neah Bay, Washington 98357 (Adjacent to the Tsoo-Yess River, Approximately 8 miles south of Neah Bay) in Clallam County, within the Makah Indian Reservation. This is a Firm Fixed Price Contract and award will be based on factors outlined in section M. The magnitude of this construction project is between $25,000 - $100,000. All questions must be submitted in writing via email to the CO Shanen Fox by the deadline outlined in section L. A Pre-Bid Site Visit is scheduled for April 12, 2022 at 10:00 AM. Refer to page 34 of this solicitation and attachment 10 for detailed pre-bid site visit info and information on the COVID-19 Attestation Form requirement, which must be completed in advance of the site visit Period of Performance: 05/16/2022 to 07/31/2022 Electronic Response. The quotation MUST be electronically submitted via e-mail to shanen_fox@fws.gov L.1.3 Requests for Information. Questions regarding this solicitation must be submitted in writing to Shanen Fox via e-mail to Shanen_fox@fws.gov by April 19, 2022 10:00 A.M. P.T. L.2.0 QUOTATION INSTRUCTIONS. L.2.1 FORMAT AND CONTENT There are three (3) non-price/cost evaluation factors and (1) price/cost evaluation factor. The technical proposal shall be organized by evaluation factor and shall not contain any price information. Pricing information to be submitted on the Quote Schedule in section B. The offeror shall address the following factors in sufficient narrative and qualitative detail to be evaluated. L.2.1.1 Submit the completed Standard Form 1442 and all acknowledged amendments, if any. L.2.1.2 Evaluation Factor 1, Past Experience. Provide details of experience (Maximum 2 Pages) working on projects of similar scope and magnitude, to include the following; a description of each project, final contract price and name of the entity you contracted with. Mentioned projects shall demonstrate past experience as a Construction Prime Contractor / Subcontractor. Provide three (3) Past Experience Projects (i.e. Recent Relevant Experience Projects) for the Construction Prime Contractor, which have occurred over the past 5 Years. Design and Construction projects of similar scope should include: Sediment Removal Work and Sediment Removal Work at or around a Pump House Intake with Operating Pumps. Evaluation Factor 2, Technical Approach. Provide a Draft Project Delivery Plan (Maximum 2 Pages). The Draft Project Delivery Plan shall outline the Construction Prime Contractor's and their Team Member's project understanding, Include a project overview, identify project stakeholders, and explain how new or emerging technologies may be implemented, which may result in demonstrated cost savings or possibly tangible benefits. The Construction Prime Contractor may present new construction methods or methodologies, which improve upon standard construction practices. At its core, the Draft Project Delivery Plan should provide an outline work plan, which right sizes the Project for construction and explains how communication and change management is part of this work plan. Lastly, the Construction Prime Contractor and their Team shall explain how current project solicitation attachments will be developed to meet project outcomes and what conditions and/or assumptions were established for the Sediment Removal Work. L.2.1.4 Evaluation Factor 3, Past Performance. (Maximum 2 Pages) Provide the Current Point of Contact (with phone number and email address) for three (3) ongoing and/or past contracts for the Construction Prime Contractor similar in scope and complexity to the work outlined in the Statement ofWork. Contractor is solely responsible for the accuracy of this information, as the Government will not pursue contact if provided with inaccurate data. Past Performance information may be collected via any other available means, such as CPARS and use of FPDS-NG, in addition to the POC's provided. The Contractor may submit the Current Point of Contacts for the three (3) Past Experience Projects submitted in Factor 1 Above. L.2.1.5 Evaluation Factor 4, Price. The Offeror shall submit the completed attached Proposal Schedule (separate attachment). The prices shall include all necessary supervision, management, labor, transportation, equipment, materials, any other directincidental costs, overhead and profit. L.2.1.6 The Respondent shall complete and submit all applicable Representations, Certifications and Other Statements of Offerors. In accordance with FAR 4.1201, representations and certifications shall be submitted to the System for Award Management (www.sam.gov) and updated at least annually to ensure they are kept current, accurate, and complete. *The value for this project is based on a financial range. The value is listed as the highest possible cost from the range provided by a stakeholder or official project document.
