Site work and paving for a road / highway in Ada, Oklahoma. Completed plans call for site work for a road / highway; and for paving for a road / highway.

Two-inch overlay project for 3.2 miles on CR 1650 (West of CR 3520) for District #3. The roadway is twenty-two (22) feet wide. This work will be performed according to the project specifi cations provided for this project. The proposal will include a special provision with reference to the Contractor's NonCollusion Affidavit and a 5% bid bond in a certifi ed or cashier's check or surety bond shall accompany the sealed proposal of each bidder. Evidence of having proper worker's compensation coverage for employees as provided by Title 85 of the Oklahoma Statutes Section 61 shall be required before issuance of a work order. Bidder questions concerning this proposal will be accepted at that time, and vendors will be given a bid packet at this time as well. Non-attendance by any potential bidder will result in the disqualifi cation of said bidder at bid opening. Upon awarding the contract to a successful bidder, the Contractor will commence work under this contract within sixty (60) days of award. The completion date for all items of work shall be thirty (30) consecutive calendar days. Pontotoc County reserves the right to reject any or all bids. Scope of Services 1. ASPHALT MIX DESIGN SHALL NOT EXCEED 25% RAP 64/22 minimum oil. 2. COUNTY will have the roads surface prepared and ready for 2.0-inch S-4 asphalt for the final driving surface with tack coat. 3. All residential and field entrances shall have 1 ft. asphalt aprons. Asphalt apron lengths may be adjusted at the discretion of on-site inspector. 4. The County shall be responsible for grading and preparation of driveways prior to asphalt paving operations. 5. The County shall be responsible for all traffic control required for construction of the project. 6. The County shall contact and coordinate with property owners/tenants any work of driveway construction requiring temporary closure of driveway. A minimum of twenty-four (24) hours notice shall be given prior to any driveway construction. 7. The contractor shall be responsible for the repair and/or replacement of all damaged property, including but not limited to structures, foundations, curb and gutter, driveways, sidewalks, fences, and landscape amenities not scheduled for removal or disturbance. 8. All work and/or materials not classified as a "Contract Pay Item" shall be considered incidental and the cost thereof shall be included in the unit price bid for the items which are classified for payment. 9. The contractor shall be responsible for any damage of private utilities. 10. The contractor shall ensure the protection and maintenance of all utilities and shall be responsible for their repair and replacement if damaged. No additional payment or time shall be considered due to any required work associated with repairs or replacement. 11. The contractor will be responsible for clean-up following completion of project. 12. The County will provide the material and shoulder up the new asphalt mat.
