Island Health is looking for consultants to provide the following professional services: 1. Surveyor, to provide a complete topographical survey of the northeast portion of the existing hospital campus as well as other tasks as requested. 2. Testing & Inspection, to provide testing and inspection services as required by code and the construction documents. In order to have your qualifications considered, your firm will need to have had significant documented experience in successfully completing similar types of services. Information submitted will be evaluated, and based upon that evaluation the most qualified firm will be selected to provide a fee proposal for the required services. Firms may submit on one or more of the above listed services, but are required to provide a separate SOQ submittal for each service. On the cover of each submittal, clearly indicate the following: the area of service of this submittal 3/4 and again only one area of service per SOQ submittal 3/4 your firm name; name, email and phone number of the contact person within your firm.
