CITY OF TUALATIN SW 95TH AVENUE, SW AVERY STREET, AND SW 93RD AVENUE Instructions for providing the First-Tier Subcontractor Disclosure and the required disclosure form are included in the Contract Documents. The disclosure form shall be submitted no later than two working hours, including lunch hours, after the Bid Closing. THE CITY WILL REJECT A BID IF THE BIDDER FAILS TO SUBMIT THE DISCLOSURE FORM WITH THIS INFORMATION BY THE STATED DEADLINE. IDENTIFICATION OF PROJECT: Major items of work consist of the following: road widening, curb and sidewalk improvements, curb ramp replacement, storm drain planter, speed humps, striping and signing, school zone flashing beacon signs, modifications to existing traffic signal and pedestrian push buttons, landscaping, and other items as called for on the plans. Schedule A includes all work shown in the Contract Documents at the intersection of SW 95th Avenue and SW Avery Street, on SW 95th Avenue, on SW Sagert Street, and on SW 93rd Avenue. Schedule B includes all work shown in the Contract Documents associated with the sidewalk replacement on the north side of SW Avery Street between SW 95th Avenue and SW 93rd Avenue. Contract Documents, including plans and specifications, may be obtained and examined at the City's bid website by creating an account and joining the plan holders list. CONTRACT TIME: The Contractor shall begin construction Work within 30 calendar days following the Notice to Proceed. Contractor shall reach substantial completion of all work in Schedule B by June 30, 2022. Contractor shall reach substantial completion of all work on SW 95th Ave and SW Avery Street and all school speed zone signage no later than August 15, 2022. Contractor shall reach substantial completion of the rest of the project no later than September 30, 2022.
