Onsite Sewage System Installation Project 134 Chilton Road Montross, Virginia 22520 Small Business Set-Aside Award Priority Scope of work: Cost for Line Item #1 (include total cost for items A-I) These are known factors. Vendors must invoice for actual cost incurred as described herein. The contractor shall furnish all labor, supervision, equipment, tools, parts, supplies and materials, as necessary, to perform the services as described herein: A) Construct an onsite sewage system that meets the location and construction specifications of the Sewage Handling and Disposal Regulations (12VAC5-610-10 et seq., the Regulations), and all other applicable state and local laws, regulations, and ordinances. B) Construct the above onsite sewage disposal system in compliance with the Westmoreland County Health Department (WCHD) Construction Permit 196-22-0048 at 134 Chilton Road Montross, Virginia 22520 in the location shown on the permit. The permit contains additional conditions, notes, and information needed to construct the onsite sewage system. C) Within 48 hours of signature of agreement to construct the onsite sewage disposal system, the contractor shall have the contents of the existing septic tank serving the subject property pumped by a properly licensed sewer hauler. The contents of the existing septic tank shall be pumped every 14 days up to one week prior to construction of the repair permit. The contents of the exiting septic tank shall be pumped every 48 hour for one week prior to installation of the repair onsite sewage system. D) As necessary, provide or subcontract with a licensed plumber and electrician to provide plumbing and electrical required to convey the wastewater from the house to the onsite sewage system as required by the Regulations, and the Virginia Uniform Statewide Building Code (VUSBC). This includes obtaining or assuring that the owner obtains all permits and inspections as may be required by the local building authority in compliance with the VUSBC. E) Abandon any unused component of the former onsite sewage system as specified by the WCHD Construction Permit 196-22-0048.The existing septic tank shall be pumped by a properly licensed sewage hauler, the tank crushed in place, lime placed over the crushed tank, and the tank hole filled with clean backfill material. F) Obtain a final installation inspection from the system designer, David Harrison, EHSS and assure that the onsite sewage system complies with the Regulations. G) Provide all required documentation to the WCHD following completion of construction and obtain an Operation Permit for the onsite sewage system. H) Follow all regulations and permitting pertaining to erosion and sediment control including stabilizing, seeding and grading the site after construction to return to the original state. Control construction runoff with proper practices so as not to become a nuisance to the owner or to neighboring properties or cause sediment to be discharged into state waters and drainage ditches. Any construction debris must also be removed from the site and disposed of properly. I) Bidders shall comply with all requirements of the Department of Professional and Occupational Regulations (DPOR) for contracting and executing the contract with the Virginia Department of Health. Must provide a copy of Class A, B, or C Contractors License from DPOR, Conventional Sewage Disposal System Contracting (CDS) specialty from DPOR, Master Conventional Onsite Sewage System Installer license from DPOR, proof of insurance. Bidders contacted by VDH to provide any missing required documents, must provide the document within 24 business hours or their bid will be considered non-responsive. Additional Services (If Needed) Bidder must contact VDH Project Manager listed on the Notice to Proceed for approval prior to any additional services performed. VDH will issue a change order for actual additional services rendered.
