Project no.1M-235-2(4585)--13-77 The City of Des Moines, hereinafter called the "Jurisdiction", has need of a qualified contractor to complete the work comprising the below referenced improvement. The undersigned Bidder hereby proposes to complete the work comprising the below referenced improvements or project as specified in the contract documents, which are officially on file with the Jurisdiction, in the Des Moines City Engineer's Office, at the prices hereinafter provided in Part C of this Proposal, for the following described improvements: 2nd Avenue Improvements from Court Avenue to 1-235, 062023007 The improvement includes Portland Cement Concrete (PCC) pavement, PCC curb & gutter, Hot Mix Asphalt (HMA), pavement removal, PCC driveways and sidewalk, storm sewer, intakes, modified subbase, pavement marking, traffic control, traffic signal modifications, trees, and other incidental items, all in accordance with the contract documents including Plan File No. 676-186/322, located on 2nd Avenue from Court Avenue to I-235 in Des Moines, Iowa. The Citys Overall Annual DBE/TSB Goal for this project is 5.72% This project shall be fully completed not later than April 15, 2027, and in accordance with the attached Completion Provisions. Each bidder shall accompany its bid with bid security as defined in Section 26.8 of the Iowa Code and as specified by the Jurisdiction. *Project information has been obtained through a public capital improvement plan (CIP).
