The City of Bakersfield (City) desires to enter into an agreement with multiple qualified maintenance and repair contractors for providing equipment maintenance and repair services on an on-call basis. The term of agreement shall be in place for one year from the effective date of the Agreement and may be extended in accordance with the provisions of this RFQ for three additional one-year periods upon City of Bakersfield approval. Projects will be maintenance and repair work within a variety of service areas. Contractors responding to this RFQ must include the following items in their submittal package: A. Three (3) copies of Proposers SOQ. B. One copy of the completed General Information Sheet (Attachment RFQ-1) signed by a company official. C. One signed copy of the Draft Independent Contractors Agreement for this project (refer to Section X, CONTRACT DOCUMENTS, of this RFQ) D. One copy of the Proposers schedule of hourly billing rates in its own sealed envelope labeled: Hourly Billing Rates. The submittal items must be placed into a sealed envelope bearing proposers name and the words Statement of Qualifications for On-Call General Services and Maintenance Public Works Department, General Services Division Bakersfield, California.
