Renovation of a communication facility in Sheboygan, Wisconsin. Completed plans call for the renovation of a communication facility.
Installation and commissioning of a fiber optic communications extension to service the new Uptown Social Community Center This is an active construction site so attendees will be required to furnish their own hard hats and safety glasses. Bidders are welcome to tour the balance of the fiber route on their own either prior to or following the meeting. A single contract will be awarded to include all facets of the project in accordance with the specifications including underground directional boring and installation of related facilities, connection and installation of the fiber into and within the building proper and testing and certification of the new fiber extension. In order to be considered, bids must be accompanied by a Bid Bond or Certified check in an amount not less than 5% of the total lump sum bid to act as surety that if awarded the work, the bidder will successfully complete the contract phase. A Performance and Payment bond in an amount not less than 100% of the lump sum bid amount will be required to be provided to the City of Sheboygan within ten days of receipt of the notice of award. All bids received must remain in effect for not less than 90 days following the due date. Attention is called to the fact that this project is funded through the American Rescue Plan Act (ARPA) grant . The bidder's attention is also called to the Notice for the Requirement of Affirmative Action to insure Equal Employment Opportunity for employment of women and minorities in the project area. In addition, it is the policy for this project that minority business enterprises shall have the maximum opportunity to participate in the performance of contracts financed in whole or in part with Federal funding under this agreement. Attention of bidders is further called to the fact that the successful bidder must ensure that employees and applicants for employment are not discriminated against because of race, color, sex, religion or national origin. Federal requirements for equal opportunity (Executive Order 11246, Segregated Facilities section 3 and section 109 and the conditions of employment and wage rates to be paid under the contract. The successful contractor shall main- tain, and furnish a certificate of insurance to the Purchasing Agent for Worker's Compensation Insurance for all workers on this contract, as well as Public Liability and Property Damage Insurance, including Contractors Contingent and Protective Insurance, as will protect him and any subcontractor performing work covered by this contract, from claims for damages for personal injury, including accidental death as well as claims for property damages which may arise from operations under this contract. The contract document will accompany a City of Sheboygan Purchase Order referencing the terms and conditions of the bid documents. In order to be considered, Bidders must have on file with the City of Sheboygan Engineering Department a Bidders Proof of Responsibility Form submitted not less than five workdays prior to the date the bids are due. Forms are included in the bid documents.All bids received shall remain in force for not less than (90) Ninety days following the bid due date. The City of Sheboygan is exempt from Federal Excise Tax and State Sales Tax. Bids shall be made exclusive of these taxes. Certification will be furnished to the successful bidder upon request. The City of Sheboygan reserves the right to reject any or all bids, cancel this solicitation, to waive informalities in the bidding process, or to accept any bid considered most advantageous to the City of Sheboygan. Construction start no later than JULY 11 TH, 2022. The substantial completion date, i.e. all duct handhole and cable placement is JULY 29 TH, 2022
