ADDENDUM TO THE SOQ This SOQ shall not be modified in any way except by a written Addendum. Any such written Addendum shall be prepared by the Commission and shall be transmitted by mail, fax, email, or other means, to each Engineer / Architect recorded by the Commission as receiving this SOQ package. Any Engineer / Architect submitting a proposal must acknowledge the receipt of any and all written Addendums in the cover letter to the SOQ proposal. EVALUATION CRITERIA AND SELECTION PROCESS The Engineer / Architect must meet the minimum qualifications and requirements set forth in the "REQUEST FOR QUALIFICATIONS" section of this SOQ. Engineer / Architects who meet the minimum qualifications and requirements will be evaluated on the following: Professional expertise (Total Combined Experience) 40 pts. Key personnel 10 pts. Demonstrated abilities 20 pts. References 10 pts. Qualifications 15 pts. Responsiveness 5 pts. A very low rating in any of the above categories will be sufficient enough cause for the rejection of the SOQ proposal. SCOPE OF WORK The Scope of Work (SOW) for the Engineer / Architect on this project will be to investigate the project site, conduct a preliminary Engineer / Architect study for the project site, develop a preliminary design including plans and specifications, develop a preliminary cost estimate for the preliminary design, develop the final design with detailed plans and specifications incorporating all changes and approvals, conduct all necessary data collection, prepare a final cost estimate, adhere to Disadvantaged Business Enterprise (DBE) requirements, and provide resident inspection services through completion. All plans and designs must be in compliance with Section 106 of the National Historic Preservation Act. The Engineer / Architect will confer with the Commission, Project Administrator (hereinafter referred to as ALL) to review all the desires and requirements of the project and scheduling will be discussed. The Engineer / Architect will furnish their recommendations to ALL. The specifications shall include General, Special, and Technical Provisions sections. The preliminary plans and specifications will go to each entity included in ALL and any corrections requested will be returned to the Engineer / Architect. The correction process will continue until the Commission furnishes the final written approval of the plans and specifications. The Engineer / Architect will be in responsible charge of the entire bidding process. The Engineer / Architect will coordinate with the local utility companies and will comply with all the necessary requirements to work in the area of the utilities. The Engineer / Architect will also be required to furnish any necessary addendums to the Final plans and specifications, as necessary, throughout the project. QUALIFICATIONS PROJECT Project Name: St. Mary Parish Water & Sewer Commission No. 4 Water System Improvements Estimated Design and Construction Costs: $6,562,892.00 Project Description: The proposed projects will facilitate upgrading the Commission Water plant, water line upgrades in the Town of Baldwin and other improvements. 1) Water Lines: a) Upgrade existing waterlines throughout the distribution system to 6" & 8" PVC waterlines. Work to include fire hydrant assemblies, valves, MJDI fittings, service connections and meter tie-ins. b) Streets: Chitimacha Trail, Utopia, Malcolm, McArthur, Marshall, Eisenhower, Rod, E. Railroad, E. St. John, Lockley, Collins, E. Long, Baptiste, Frontage, 8th, 9th, lOth and Jolivette. 2) Increase Capacity and Improvements to Raw Water Supply, Treatment Plant, Storage and related Items to Accommodate Consolidation with the Town of Baldwin. QUALIFICATION REQUIREMENTS The qualifications shall be submitted in the form of a SOQ Proposal. The SOQ Proposal must include the following information. Cover Letter include the name and contact information of the contact person for the Engineer / Architect Firm; and include acknowledgement of any addendums issued by the Commission. Project Approach Provide a description of the Engineer / Architect's approach to develop the project design, plans, specifications, and other project tasks included in the SOW. History of the Firm Provide a detailed history of the Engineer / Architect Firm and prior related experience. Related Experience Engineer / Architect must show a detailed description of experience in mitigation projects, preferably within the State of Louisiana, that include structure elevation and wind retrofit. Bidding and Management Experience Engineer / Architect must show experience in the bidding process and project management experience. Engineer / Architect & Support Staff Background Provide detailed biographical information on the individual who will manage the Commission's account, including education, training, and experience; and provide biographical sketches of staff members who will assist the account manager. Equipment Provide a detailed inventory of the owned/leased/rented equipment that the Engineer / Architect Firm will have access to and utilize in order to complete the site investigations, data collections, design and plans development, and specifications. Personnel Provide detailed description of the qualifications of the personnel that will be involved in running the necessary equipment provided in the equipment list. References Provide a list of the names, addresses and phone numbers of at least five (5) professional references for whom the Engineer / Architect Firm has provided professional Engineer / Architect services. With the list, include the professional services provided to the listed reference. Qualifications Provide a detailed description as to how the Engineer / Architect Firm meets the requirement of being a full-service firm with a very high degree of professionalism and significant experience within area of the project SOW. Other Include any other pertinent information that may be used by the Commission to elevate the Engineer / Architect's SOQ proposal.
