The City of Athens is currently soliciting quotes to install drainage pipe, grade drainage ditch, install stormwater inlet box, and create a junction and cleanout area in an existing 72-inch reinforced concrete pipe, located on the alley from Stirman Street to Pinkerton Street. Provide lump sum competitive pricing and a detailed schedule & timeline that meet the Minimum Specifications listed below. MINIMUM SPECIFICATIONS:
o Follow the attached engineer drawings for grading and new installation.
o Furnish all equipment and materials to complete the job.
o Dispose of any vegetation, and or brush as well as any soil that may be generated from this project.
o Coordinate with the homeowner for access.
o The City's Engineer will stake grade stakes and set elevations for flowlines. INSURANCE REQUIREMENTS
o Insurance is required in the performance of this work. Contractor will be required to have and maintain
Statutory Workers' Compensation Insurance, and Commercial General Liability insurance with limits
of not less than $500,000 per occurrence and $1,000,000 aggregate prior to starting work.
o The City reserves the right to establish that these insurance requirements are met and complied with
prior to any contractual agreement being entered into. The cost of such insurance is the service
provider's responsibility. Failure to comply could result in non-award of the contract. REQUIREMENTS:

Upon award Vendor agrees to complete the attached Conflict of Interest Questionnaire, IRS Form W-9 and, where applicable, to provide a Certificate of Insurance with the City of Athens listed as Certificate Holder. Forms must be approved by the City prior to the issuance of a Purchase Order or other form of purchasing agreement.
