Site work for a water / sewer project in De Soto, Wisconsin. Completed plans call for site work for a water / sewer project.
Construction project to furnish all labor, equipment, and materials necessary to replace a 1,000-gallon septic holding tank located in a volunteer/contractor recreational vehicle (RV) site. The Contractor shall remove and dispose of the existing concrete holding tank. The Contractor shall procure and install a new , precast 1,000-gallon concrete holding tank w ith ballast, and all attachments needed for a standard RV hookup w ith a 4" PVC pipe in accordance w ith scope provided herein or as amended. The North American Industry Classification System code for this project is 238910 Site Preparation Contractors w ith a size standard of $16.5M. This solicitation is issued as a Request for Quote (RFQ) under the authority of FAR Part 13 Simplified Acquisition Procedures and is 100% set aside for Small Business. The estimated magnitude of this construction project is less than $25,000. SUBMITTAL INSTRUCTIONS This solicitation is a Request for Quote (RFQ), which should result in one firm-fixed price contract. This solicitation is not an Invitation for Bid (IFB) or a Request for Proposal (RFP). Any questions concerning this RFQ shall be accepted via email to toni.w.wasgatt@usace.army.mil. Quotes shall be evaluated by price and past performance. Quotes must contain the following: 1. Complete SF 1442 Blocks 14, 15 and 20 a-c (with signature in Block 20b). 2. Complete pricing for both Contract Line Items (CLINS) 0001 and 0002 to include supply, delivery, and installation. 3. Complete FAR 52.204-8 Annual Representations and Certifications. 4. A copy of the manufacturer's cut sheets or drawings with the make and model of the Septic Holding Tank. 5. Provide up to five past performance references. Information should include contact information and reference the contract number of any government contracts. All pricing (unit, total, subtotal, grand total, etc.) must be in whole penny amounts. Pricing must be provided for both CLINS. Offeror shall be registered in the System for Award Management (SAM) https://www.sam.gov to include the FAR and DFARS Representations and Certifications section, with the NAICS Code 238910 Site Preparation Contractors. Quotes received after that date and time are late and may not be considered for evaluation and contract award. It is the responsibility of the firm to specifically acknowledge all amendments to this solicitation with its quote. The preferred method of amendment acknowledgement is to return a signed copy of the amendment's (Form SF30) first page as a .pdf document attached to the quote. Failure to acknowledge amendments may results in the quote's removal from consideration for contract award. As a component of the U.S. Government, the U.S. Army Corps of Engineers is exempt from Federal, State, and Local Taxes, including sales taxes. Do not include sales tax(es) in quoted price. SCOPE OF WORK 1. SCOPE: The U.S. Army Corps of Engineers, St. Paul District (Corps), Blackhawk Park Recreation Area requires a Contractor to furnish all labor, equipment, and materials necessary to replace a 1,000-gallon septic holding tank located in a volunteer/contractor recreational vehicle (RV) site. The Contractor shall remove and dispose of the existing concrete holding tank. The Contractor shall procure and install a new, precast 1,000- gallon concrete holding tank with ballast, and all attachments needed for a standard RV hookup with a 4" PVC pipe. 2. LOCATION: The Corps' Blackhawk Park is located three (3) miles North of DeSoto, Wisconsin, off of State Highway 35. 3. SITE VISIT: Interested Contractors are encouraged to visit the site prior to submitting a quote. To schedule a site visit, please contact: Mr. Eric Hammer 651-290-5996 office 608-648-3314 cell eric.m.hammer@usace.army.mil 4. SCHEDULE: a. Project Completion: Friday, 18 November 2022. b. All work shall occur during Blackhawk Park's operation hours of 8:00am to 5:00pm, Monday through Thursday. Because this is a busy recreation area, no work shall occur on Fridays, Saturdays, Sundays, or Federal Holidays without permission from the Contracting Officer. c. A Pre-Construction Meeting with Corps and Contractor shall occur as soon as possible after contract award. All required submittals, to include planned work schedule, shall be submitted to the Corps in writing prior to the Pre-Construction Meeting. Discussion of the general execution of the work shall be discussed at this meeting. 5. REMOVAL AND INSTALLATION: The location of the subject septic holding tank is marked on the photo below at "Volunteer Site 001", in the upper loop campground. a. Removal of Existing Tank: 1) The existing septic holding tank is estimated to be a 20ft x 10ft, 1,000-gallon, concrete tank that is located at a depth of approximately 50 inches below ground level. 2) The existing septic holding tank shall be removed and disposed of in accordance with all local and state regulations. Due to the area's cultural significance, the existing septic holding tank must not be collapsed in place. The new septic holding tank shall be placed in the existing tank's location. 3) The Corps will be responsible for emptying the existing septic tank prior to commencement of its removal. b. Installation of New Tank: 1) The new, precast concrete septic holding tank shall be approximately the same size as the existing tank to fit in the existing location. It shall be installed in accordance with local and State of Wisconsin standards and manufacturer's specifications. The new tank will be placed in a location that the water table/river levels may become higher than the tank cover. A tank ballast is required. W912ES22Q0077 Page 4 of 54 2) The Contractor shall supply and place any and all fill/aggregate materials needed for bedding and backfill to properly install the new tank in accordance with the tank manufacturer's instructions. 3) The new tank shall be installed and ready for full use with a standard 4" PVC pipe RV hookup. 6. TRAFFIC CONTROL: The construction project area must not be accessible to recreation area visitors. Sections of the access road may be temporarily closed during construction after coordination with Corps' site staff. The Contractor shall be responsible for all applicable warning and closure signs and must provide flagging services as necessary to safeguard the public. 7. SAFETY: All project work shall be performed in accordance with the Corps' Safety Manual EM 385-1-1. All project work areas, attended or unattended, shall contain proper safety hazard signage. The Contractor shall plan and execute all work in a manner that minimizes interruption of the park's normal traffic flow and prevents injury to trees and appurtenant structures. 8. PAYMENT: A lump sum payment shall be made upon satisfactory completion of the construction project. The U.S. Army Corps of Engineers is tax exempt.
