The Township of Grosse Ile, Michigan ("MI"), through its Board of Trustees ("Board") is requesting proposals for the construction of multiple foundations (bases) along Macomb St. in Grosse Ile Township, MI 48138 outside of the road right-of-way, for the purpose of installing art structures of various shapes and sizes. Work to include excavation, forming, reinforcement, all material, equipment, and labor to provide for an installation ready for use by the owner. The scope of the project in general consists of the following: -The construction of 37 art structure foundations (bases) -Restoration of all disturbed areas All proposals shall include, as a minimum, the following provisions and information: 1. All of the significant assumptions on which the pricing is based. 2. A total project cost to the Township of Grosse Ile for the installation of 37 foundations of various shapes and sizes. 3. A schedule detailing Project permitting, design, construction, start, and finish date projections; 4. A list of Personnel to be assigned to the Project, including all qualifications. 5. The ability to provide General Liability and Products/Completed operation aggregate of $2,000,000 and excess Liability insurance of not less than $2,000,000 along with Worker's compensation insurance. Also, XCU (explosion, collapse, underground) coverage shall be provided. Proof of coverage must be provided with Proposal. 6. Full company name, tax identification number, main office address, telephone number and signed by a corporate officer. 7. Have specific experience in the State of Michigan with similar type projects. 8. Identification of the firms that the respondent intends to subcontract or otherwise use to perform work on this project. 9. The ability to furnish all labor, material, and equipment necessary for all the preparatory work and operations, including but not limited to those necessary for the movement of personnel, equipment, supplies, and incidentals to the project site for Contractor facilities necessary to undertake the work on the project, and for expenses incurred, prior to beginning work on the contract. This shall also include all preconstruction costs, including insurance and bonds, exclusive of quoting costs, which are necessary direct costs to the project and are of a general nature. All costs necessary to accomplish the above for an installation ready to use by the Owner shall be included. 10. Labor, maintenance, and performance bonds in the amount of not less than 100% of the contract price Work must be substantially completed on or before Tuesday, November 8, 2022 with Final completion on or before Tuesday, November 15, 2022. THE CONSTRUCTION OF 37 ART FOUNDATIONS (BASES) RESTORATION OF ALL DISTURBED AREAS Construct 37 round art foundations varying in diameter from 2.5' to 3' and in height from 4' to 7' including but not limited to the following: 1. 4000 PSI air entrained concrete 2. Reinforcement cages including straight # 4 bars and round # 3 stirrups 3. Sonotubes for forming 4. Excavation by augering 5. 1" diameter, 18" minimum length anchor bolts 6. Layout 7. Restoration with seed and mulch 8. All work, labor, material etc. necessary to provide the owner with foundations ready for use
