Mesa County PublicLibrary ( the Owner) is seeking qualified construction materials testing firms to perform standard observations and testing for the project listed below. The new Mesa County Clifton Branch Library is to be built just east of Rocky Mountain Elementary, at 3270 D 1/2 Rd, Clifton, CO 81520. The new facility will be an approximately 18,000 SF building that was designed by Ratio Architects and being built by the Construction Manager at Risk (CMAR) FCI Constructors. The Project construction documents are completed and the CMAR is finalizing the bid and contract phase and is mobilizing on site with construction scheduled to start at the end of August. The Project is scheduled to be completed in June 2023. The Project does not have any specific sustainability or LEED requirements. Proposals must include, but are not limited to, the following items: o Letter of interest, including address, telephone number, and email address, and signed by the principal-in-charge. o Written statement of qualifications with the following information: o Provide number of years in business as a Materials Testing firm for construction projects. Identify location and size of office(s). o Identify the key personnel that will be assigned to this project. Include all relevant experience. Include resumes with work experience for all personnel that have been identified and include a list of specific responsibilities for each. References - Submit a minimum of three references with email and phone number. Description of five projects of similar scope/complexity completed by proposing firm. Include the names of the key personnel. Litigation and termination history. Both of the following: Completed Materials Testing Proposal Form, Exhibit A (below) Materials Testing Estimate Worksheet, Exhibit B. Insurance certificates naming the Owner as additional insured will be required prior to work commencing, but not required as part of this submittal. See "EXHIBIT C: Sample Consultant Agreement" for insurance requirements. Use the included proposal forms to provide firm information and personnel rates. o Use the attached excel workbook to provide bids for the project.
