Renovation of a mixed-use development in Whiteriver, Arizona. Conceptual plans call for the renovation of a medical facility.

The Whiteriver Indian Health Service Hospital requires a contracted project for the correction of fire sprinkler inspection deficiencies including (1) installing a fire sprinkler in an uncovered office and (2) adjusting a sprinkler head that had dropped below the ceiling tile. A. BACKGROUND The Whiteriver Indian Health Service Hospital requires a contracted project for the correction of fire sprinkler inspection deficiencies including (1) installing a fire sprinkler in an uncovered office and (2) adjusting a sprinkler head that had dropped below the ceiling tile. The fire sprinkler addition is located at the Whiteriver Indian Hospital Annex (Garrett Building) in Whiteriver, Arizona. The fire sprinkler adjustment is located at the Cibecue Health Clinic at Cibecue, Arizona. The scope of work detailed below outlines the required project outcomes. The Contractor shall be responsible for all necessary work to achieve required outcomes including labor, equipment, tools, personal protective equipment (PPE), policy adherence, infection control compliance, incidental design, travel, per diem, and other related expenses required to complete the project. B. SUMMARY 1. Fire Sprinkler Addition: a. The Whiteriver Hospital Annex 8' square foot (SF) x 11 SF engineer office currently does not have fire sprinkler coverage. The annual fire sprinkler inspection has identified this as a deficiency and requires correction for code compliance. The office was constructed as an addition in 2019 without fire sprinkler coverage. See Attachment A for photograph. 2. Fire Sprinkler Adjustment: a. The Cibecue Health Clinic fire sprinkler line is drooping above the Medical Records Office resulting in the fire sprinkler being identified as a deficiency for dropping below the ceiling tile. Line adjustment has been attempted without success. See Attachment B for photographs. C. DELIVERABLES 1. Fire Sprinkler Addition: a. The contractor shall install a new fire sprinkler in the engineer office to include extending the fire sprinkler line. The closest fire sprinkler line to the office is about 14'. The contractor shall verify all measurements. b. The contractor shall connect to the existing exposed fire sprinkler system. c. The contractor shall penetrate the engineer office hard lid ceiling and patch the drywall as needed to seal the opening, if larger than the sprinkler escutcheon. 2. Fire Sprinkler Adjustment: a. The contractor shall raise the sprinkler so that the top of the sprinkler is flush with the ceiling tile either by adjusting/raising the sprinkler line or shortening the sprinkler head pipe length. b. The contractor shall comply with Infection Control requirements. The Infection Control Policy is located at Attachment C. D. PERIOD OF PERFORMANCE 1. The period of performance of this contract is 30 calendar days after award. 2. Facility day time hours of operation are 8:00 AM - 4:00 PM, Monday through Friday (excluding Federal holidays). Work outside of these hours must be requested in writing to the Contracting Officers Representative (COR). E. PLACE OF PERFORMANCE: 1. Fire Sprinkler Addition: All work shall be performed at the Whiteriver Indian Health Service Hospital Annex (Garrett Building) located at 400 W. Apache Drive, Whiteriver, Arizona 85941. 2. Fire Sprinkler Adjustment: All work shall be performed at the Cibecue Health Clinic located at W. Cromwell Road, Cibecue, Arizona 85911. F. CODES AND STANDARDS 1. The Contractor is responsible for securing all required permits and for ensuring compliance with all federal, state, tribal, and local codes and design standards, latest editions. 2. The Contractor shall adhere to the following code requirements for work performed, testing and inspections, including, but not limited to the Occupational Safety and Health Administration (OSHA) regulations, National Fire Protection Association (NFPA) standards NFPA 70, NFPA 99, NFPA 101, and NFPA 111. 3. If discrepancies are found between the scope of work and codes or standards as referenced in this SOW, the codes or standards shall govern, unless noted otherwise. G. ADDITIONAL REQUIREMENTS 1. Safety a. All work will comply with Occupational Safety and Health Act (CFR1926). b. Contractor shall supply all required worksite safety and personal protective equipment. c. Contractor shall remove and dispose of excess materials, debris, or waste at an approved offsite location in accordance with applicable local, Tribal, state and federal laws and regulations, and pay any related fees. d. Housekeeping will be accomplished on a daily basis. All debris and trash generated by this project will be disposed of properly in accordance with governing national, state, and local environmental regulations. 2. Warranty: The Contractor shall provide the Government a minimum one (1) year written warranty on the building and equipment against defects in material and workmanship. Any repairs necessary shall be made seventy-two (72) hours after the notification to the Contractor by the Phoenix Area Office. The warranty period to commence upon formal acceptance by the Government. The method of contractor selection has not been determined at this time
