A. BACKGROUND The Whiteriver Service Unit (WRSU) requires a 5-year (base year plus four option years) contract for seven housing asbestos-containing-material (ACM) abatements per year. The scope of work detailed below outlines the required project outcomes. The Contractor shall be responsible for all necessary work to achieve required outcomes including labor, equipment, tools, personal protective equipment (PPE), policy adherence, travel, per diem, tribal fees, and other related expenses required to complete the scope of work. B. SUMMARY Various housing quarters at the WRSU Medicine City housing unit complex require ACM abatement in the kitchen, hall bathroom, hall utility closet, and/or master bedroom bathroom. The housing unit(s) to be abated shall be identified in each task order. The sheet vinyl flooring in these rooms contains asbestos containing material (ACM). The sheet vinyl tile contains homogeneous, nonfriable, nonfibrous chrysotile asbestos with content between 5-20%. This information was extracted from the 2007 survey report developed by the U.S. Public Health Service. In multiple rooms, the ACM sheet vinyl tiling is encapsulated in a second layer of vinyl tiling. Both layers require removal and disposal during the abatement process. C. DELIVERABLES 1. Housing Unit: The Contractor shall abate the following rooms: kitchen (126.5SF), hall bathroom (30SF), master bathroom (25SF), and hall utility closet (48SF). The furnace, water heater, bathtub, cabinet, and toilet will be removed by the government prior to arrival of the abatement contractor. The contractor shall remove the top layer of sheet vinyl tiling then remove the asbestos containing sheet vinyl tile under the upper layer. See Attachment A for photographs of typical rooms. ACM abatement shall be performed by appropriately trained and certified personnel. Removed flooring shall be disposed in accordance with Environmental Protection Agency (EPA) requirements for asbestos waste. Following the ACM removal, the contractor shall perform tests to ensure the abatement process was successful. 2. Report Submission: The contractor shall perform testing and provide the test results showing that the ACM materials were completely removed. D. PERIOD OF PERFORMANCE Facility day time hours of operation are 7:00 AM - 5:00 PM, Monday through Friday (excluding Federal holidays). Work outside of these hours must be requested in writing to the Contracting Officers Representative (COR). E. PLACE OF PERFORMANCE All work shall be performed at the Whiteriver Service Unit (WRSU) Medicine City Housing Campus located at 200 West Hospital Drive, Whiteriver, Arizona 85941. WRSU Housing Unit Abatement Whiteriver Service Unit F. CODES AND STANDARDS 1. The Contractor is responsible for securing all required permits and for ensuring compliance with all federal, state, tribal, and local codes and design standards, latest editions. 2. The Contractor shall adhere to the following code requirements for work performed, testing and inspections, including, but not limited to the Occupational Safety and Health Administration (OSHA) regulations. G. ADDITIONAL REQUIREMENTS 1. All work will comply with Occupational Safety and Health Act (CFR1926). 2. Contractor shall supply all required worksite safety and personal protective equipment. 3. Contractor shall remove and dispose of excess materials, debris, or waste at an approved off-site location in accordance with applicable local, Tribal, state and federal laws and regulations, and pay any related fees. The Contractor shall provide all required waste storage containers and coordinate their location on site with the COR. 4. Housekeeping will be accomplished on a daily basis. All debris and trash generated by this project will be disposed of properly in accordance with governing national, state, and local environmental regulations. H.
