Renovation of an educational facility in Normal, Illinois. Completed plans call for the renovation of a educational facility.
Scope of Project: Provide all labor, material, equipment, and services necessary to remove all delapidate concrete and structural issues with the sub-basement stairs and replace with new concrete and reinforcing steel to provide safe passage, at the Linkins Dining Center on the campus of Illinois State University. Work shall also include the replacement of all doors (3) and repainting of all existing guardrails and handrails and associated work. Bids shall include the following: 1. Bid security in the form of a bid bond, certified check, cashier's check, or bank money order, payable to the Board of Trustees of Illinois State University, in the amount of: General $ 5% of bid 2. A letter from a surety company in substantially the form set forth in the Instructions to Bidders assuring the issuance of the Performance and Payment Bond. 3. One (1) completed copies of the Illinois Department of Human Rights Form PC-2, entitled "Bidder's Employee Utilization Form Construction". 4. Form A or Form B: Certifications & additional terms, Disclosures and Conflicts of Interest 5. Vendor/Contractor use of Subcontractors 6. BEP Utilization Plan Bids may not be withdrawn after the opening of bids.
