This RFQ is for material for West Outer Drive Water Main Replacement. Any additions, deletions, or variations from the attached specifications must be noted. If quoting an Equal or Substitute Provide complete product description and specifications, including manufacturer, model number, etc. and provide necessary back-up literature if applicable. Your bid must include the following: e Itemized prices, extensions and bid total e Earliest firm delivery date (delivery dates may be a factor in the award process) e Quote delivered price F.O.B. Oak Ridge or notate ESTIMATED freight charges e Supplier's name, address, phone #, email and authorized signature Bids submitted should indicate on the outside envelope, lower left corner, the following: Sealed Bid RFQ # 177636 to ensure it is delivered to the appropriate person at the City in a timely fashion. All bids shail be completed and submitted on the Bid Form provided and submitted on or before the time set for the opening of bids. Bids received after the time so set are late bids and will not be considered. Late bids, unmarked envelopes, and incorrectly marked envelopes will not be opened nor returned. No Electronic (emailed), telephoned or faxed bids will be accepted. The City of Oak Ridge reserves the right to reject all, or any part of any bids submitted.
