For South Superior Street Improvements - Provide all exterior concrete flatwork, including concrete paving, walks and curbs, as shown on the Drawings or specified herein, or as required to complete the Work. -includes excavating for manhole I catch basin construction, installing concrete of classes called for, brick, Portland cement mortar, reinforcing steel, pre-cast concrete pipe, integral base sections, riser sections, transition sections, eccentric cone sections, flat slab tops, adjusting rings, flexible manhole I catch basin connections, pipe for drop connections, plugging lifting holes, pointing joints, forming channels through manhole / catch basin bottoms, making watertight connections to new and existing sewers, and other work incidental to manhole / catch basin construction. the Contractor shall furnish and install chip seal or hot-mix or cold-mix asphaltic concrete paved traffic, road and parking areas, as shown on the drawings or specified herein, or as required to complete the work. A bid bond or cashiers check in the amount of ten percent (10%) of the total bid amount shall accompany each proposal submitted. Upon entering into a contract with the Owner, the contractor must then file a bond for the amount of the contract, and the check will be returned to the successful and unsuccessful bidders when the contract is executed. The successful bidder will be required to sign an affidavit of contractor or supplier of non-delinquency of personal property taxes in accordance with Section 5719.042, Revised Code of Ohio and an Affidavit Regarding Child Support, pursuant to the Board of County Commissioners Resolution No. 92-2041.
