3. All certificates must designate the City of Saginaw as the Certificate Holder and must be on file in the Purchasing Office prior to the bid opening.
4. Certificates must be addressed as follows: City of Saginaw, ATTN: Purchasing, 1315 S. Washington Ave., Saginaw, MI 48601.
5. All certificates must include a clause stating that the City of Saginaw, as Certificate Holder, will receive a minimum 30-day written notice of cancellation or termination of the policy for any reason.
6. All certificates of insurance must be signed and mailed or faxed directly from the issuing agent. Hand delivered certificates from vendors will not be accepted. All successful bidders will be subject to income tax withholdings and compliance is mandatory. An original bid bond, certified check or cashiers check in the amount of five percent (5%) of the total amount bid must be submitted with any bid in excess of $100,000. If agreement is for multiple years, each year should be combined to get your total cost. Facsimiles or copies of the bid bond will not be accepted and your bid will be disqualified if submitted. The city reserves the right to accept or reject any and all bids, or parts thereof, and to waive any irregularities which do not materially affect the bid document or otherwise conflict with statute or ordinance. Any rejections of bids shall be made only if there is a sound documented reason for same.
