Perryopolis, PA 15473 Contracts 01 General Construction 02 HVAC Construction 03 Electrical Construction Each Bid shall be submitted in a sealed, opaque envelope and individually marked with the following Project Name (as noted above) Prime Construction Contract Number and Name (eg. 03 HVAC Construction ) Company Name Bid Enclosed Do Not Open Bids shall be made upon the form provided and shall be accompanied by a bid bond, certified check, or cashier s check in the amount of ten percent of the Base Bid amount made payable to or indemnifying the Frazier School District. In addition, submit the following with each bid: Non-Collusion Affidavit, Contractors Qualification Statement, and Certificate of Authority (if applicable). Owner reserves the right to waive any informalities in bids and to accept or reject any or all Bids.
