All questions pertaining to the general nature ofthis project or to the specific requirements ofthis RFQ must be received in writing and may be addressed to the Project Manager listed in the Contact Information ofthe front page ofthis RFQ. All addendum that may be issued for this RFQ shall be acknowledged and to be included with the RFQ submission. The following are the General Conditions for the Work to be performed as outlined in the Specifications. The Contractor shall furnish all equipment, tools, labor and materials to complete the project in accordance with Specifications. Remove 9 existing skylights and dispose Remove existing asphalt shingles around the 9 skylights and dispose Contractor is responsible for all measurements to install skylights in existing openings. Replace with nine (9) Curb-Mounted Velux Skylights or equal. Contractor shall install the 9 skylights per the manufactures Curb Mounted instructions. Replace asphalt shingles with new shingles around the skylights to match the color of the existing shingles Adhesive underlayment shall be installed for secondary water protection around new' curbs for each skylight per the manufactures instructions. Flashing kits shall be installed for primary water protection on all 9 skylights Contractor shall finish trim side ofskylights to match existing trim. Contractor shall provide a work zone area to ensure the safety ofthe public and staff. Contractor shall water test all newly installed skylights to ensure no leaks. All work will require submittal of an application to perform building work and inspection during the work. Building permit is the responsibly if the contractor to obtain. There will be no cost for the building permit. All work must pass inspection All work must follow OSHA regulations Contractor is responsible for all debris cleanup during and after work preformed The Town of Yarmouth is seeking written quotes for the removal of 9 existing sky lights and replacement with 9 new curb mounted Velux skylights (or equal) for the Yarmouth Senior Center located at 528 Forest Road, South Yarmouth, MA 02664. This project is time sensitive and should be completed before winter. This quote request shall be in accordance with Massachusetts General Law, Chapter 149 standards.
