Renovation of a municipal facility in Perth Amboy, New Jersey. Completed plans call for the renovation of a 8,628-square-foot, three-story above grade municipal facility.
GENERAL DESCRIPTION OF WORK: the work includes, but is not limited to, selective interior demolition of approximately 2,200 square feet of existing floor and ceiling finishes, plywood floor substrate, casework, and non-bearing partitions. Construction of new public service counters and staff work stations. Installation of new plywood floor substrate, LVT, carpet, and a new suspended ceiling system with new paint finishes throughout the work area. Electric work to include new power and light fixtures, network cabling and fan coil, modification to existing fire alarm system. Mechanical work will include installation of unit heaters, re-installation of existing ductwork to new ceiling grilles. LOCATION OF WORK: 260 High Street Perth Amboy, New Jersey 08861 BID SECURITY: Bid Security is required in the amount not less than ten percent (10%) of the total amount indicated in the Bid, but not in excess of Twenty Thousand Dollars ($20,000.00). No bid may be withdrawn for a period of sixty (60) days after the date set for opening except with express written approval by the Owner. Each bid proposal must contain an original and one (1) copy of the following: 1. A Bid Security in the form specified in the Bidding Documents to the order of the Union Beach Board of Education, in an amount of not less than ten percent (10%) of the total amount of the bid, not to exceed Twenty Thousand ($20,000.00) Dollars; 2. A commitment of Surety evidencing that the Successful Bidder for any Contracts) or Combined Contract will furnish in the form required for a surety company satisfying the requirements of the Bidding Documents including: A performance Bond in the amount of One Hundred Percent (100%) of the Contract Price, A Labor and Materials Bond in the amount of One Hundred Percent (100%) of the Contract Price; and, at the time required in the Bidding Documents, a Two-Year Maintenance Bond for the total sum of the Contract Price; 3. Completed and executed Proposal Bid Form; 4. The identity of any subcontractor proposed by the Bidder for use on any portion of the Work which is required to be disclosed by N.J.S.A. 40A:ll-16; 5. Corporate Disclosure Statement, pursuant to P.L. 1997, C.33; 6. Acknowledgement of Receipt of any Changes to Bid Documents NJSA 40A:ll23.1; 7. Bid Document Submission Checklist; 8. Non-Collusion Affidavit; 9. Exhibit B- Mandatory Equal Opportunity Language; 10. Americans with Disabilities Act of 1990; 11. Certification of Compliance with the City of Perth Amboy's Local Pay to Play Ordinance; 12. Contractors Qualification Statement; 13. Public Works Contractors Registration Act Certificate; 14. State of New Jersey Business Registration Certificate; 15. Contractor's Sworn Statement; 16. Sub-Contractor's Sworn Statement completed by identified designated subcontractors pursuant to N.J.S.A. 40A: 11-16. Disclosure of Investment Activities in Iran. CONSENT OF SURETY: Certificate from a Surety Company is required indicating consent to be bound as surety and guarantor for performance required under the Contract Documents. STATUTORY REQUIREMENTS: The requirements of New Jersey Prevailing Wage Act (N.J.S.A. 34:11-56.25, et seq.), Affirmative Action Act (N.J.S.A. 10:5-31, et. seq.), N.J.A.C. 17:27-1, et. seq., The Public Works Contractor Registration Act (P.L. 1999, c. 238) and all other applicable law shall be fulfilled. OWNER'S DISCLAIMER/ OWNER'S DISCRETION TO ACCEPT OR REJECT BIDS: The Owner assumes no responsibility, whatsoever, in connection with any defects arising out of the Bidding Documents or the receipt or failure to receive Bids, including those which may arise from delay for any reason in obtaining or submitting the Bidding Documents including but not limited to, traffic delay, messengering, mislabeling, misdirections from any source, misdelivery or otherwise. TIME FOR MAKING AWARDS, DEPOSITS RETURNED: The Owner will award the contract or reject all bids within sixty (60) days of receipt, except that the bids of any bidders, who consent thereto may, at the request of the Owner, be held for consideration for such longer period as may be agreed. All bid security except the security of the three apparent lowest responsible bidders shall, if requested, be returned after ten (10) days from the opening of the bids, Sundays and holidays excepted and the bids of such bidders shall be considered as withdrawn. Within three (3) days after the award of the contract and the approval of the contractor's performance bond, the bid security of the remaining unsuccessful bidders shall be returned to them forthwith, Sundays and holidays excepted. REJECTION OF BIDS: The right is reserved by the Owner to reject any or all bids or to waive any immaterial defect or informality in any bid, if deemed in the best interest of the Owner. Any Bid which does not fulfill the material requirements of the Bidding Documents will be rejected. When it is in its best interest to do so, the Owner may award the Contract in whole or in part, select any combination of bids, or may waive any informalities in or reject any or all bids.
