The City of Moline Public Works Department is requesting proposals for a contractor to provide all necessary work, labor, services, transportation, equipment, materials, apparatus, machinery, tools, waste disposal and other means and items necessary to remove waste and debris from two locations. The intent is to remove waste and debris from the BUILDING INTERIORS ONLY, INCLUDING THE MAIN FLOOR, UPPER FLOOR(S) AND ALL STAIRWELLS ONLY, NOT THE BASEMENT at recently acquired properties in Moline to prepare for possible redevelopment of the buildings Services shall be rendered to the following locations: 1. McKinley School 4108 5th Ave, Moline, IL 61265 2. Moline Manual Arts 855 17th Street, Moline, IL 61265 1. Contractor shall remove waste and debris at the two (2) aforementioned City of Moline buildings (MAIN FLOOR, UPPER FLOOR(S) AND STAIRWELLS BETWEEN FLOORS ONLY - DO NOT GO INTO BASEMENT). 2. Contractor shall haul the waste and debris to an approved landfill and/or recycling facility. 3. Contractor shall be responsible for complying with all Federal, State and local laws and ordinances related to the disposal of the waste and debris. 4. Contractor shall be responsible for any damage to public and/or private property caused during the cleanup operation. 5. Empty all trash containers and receptacles 6. Remove any hanging, partially attached pieces of ceiling, floor and walls in a professional manner in accordance with standard construction practices. 7. Working hours, unless approved in writing by the Department of Public Works. 8. Dumpsters and refuse containers shall be removed expeditiously when full and shall be removed permanently prior to final walk-through. 9. Any food, animal waste or other putrescible material shall be removed from the site and disposed of in strict accordance with local, county, state and federal law. 10. Floors shall be swept clean and mopped to remove dust and other particles. 11. Buildings are to remain secured at all times except for limited access for the contractor. a. Any evidence of entry by a person or persons other than the contractor shall be immediately reported to the Department of Public Works or the Police Department, if after normal working hours. 12. The contractor shall use adequate lighting, including but not limited to all stairs, openings such as loading docks or any elevated platforms. 13. Smoking is not allowed at any time while inside the building or within 15 feet of any structure. 14. The contractor shall, upon entry and during all times while in the structure: a. Provide and use proper PPE for all employees, including subcontractors, material suppliers and anyone else who may enter the building b. Properly cover any holes in floor and prevent any slip, trip and fall hazards c. Remove any sharp or dangerous items d. Notify the city and properly dispose of any needles, syringes or other potentially hazardous or biohazardous materials e. Notify the city of any comingled asbestos f. Notify the city of any potential electrical hazards g. Provide, at no cost to the city, any generators or other means of power used for lighting, fans, power tools or other devices used during the cleanout h. Notify the city immediately of any potential fire hazard Questions regarding the proposal process or submitting your proposal should be directed to: Mike Doi, Director of Public Works at (309) 524-2346 or email: mdoi@moline.il.us Proposals must be submitted on the required forms and signed by an authorized agent to be considered responsive to the request for proposals. The City of Moline reserves the right to accept or reject any or all proposals, decide what products meets, exceeds or are equal to specifications and to waive any technicalities. Pursuant to the Moline Code of Ordinance Section 27-3102, the City of Moline reserves the right to cancel the proposal request any time prior to the acceptance of the proposal. The following items must be included with your proposal or be subject to disqualification: 1. Proposal Specification Sheet - Specifications are a minimum and if your proposal exceeds the specifications, then you are in compliance. 2. EEO Policy Statement - Sign and date this page and include your own, if applicable. 3. Contractor/Supplier Certification - All proposals must be accompanied by a completed Contractor/Supplier Certification, pursuant to 720 ILCS 5/33E-11. Any questions concerning the requirements of the specifications and/or work shall be addressed in writing to Director of Public Works Mike Doi at 3635 4th Avenue, Moline, Illinois. In the event any addenda are issued, they will be mailed or delivered to all who are known by the issuing office to have received a complete set of proposal documents. Copies of addenda will also be made available for inspection wherever proposal documents are on file for that purpose. No addenda will be issues later than two business days prior to the date for receipt of proposals except an addendum withdrawing the request for proposals or one which includes postponement of the date for receipt of proposals. Contractor shall pay and require every Subcontractor to pay prevailing wages as established by the Illinois Department of Labor for each craft or type of work needed to execute the contract in accordance with 820 ILCS 130/.01 et seq. Any increases in costs to the Contractor due to changes in the prevailing rate of wage during the terms of any contract shall be at the expense of the Contractor and not at the expense of the City. The change order shall be computed using the prevailing wage rates applicable at the time the change order work is scheduled to be performed. The Contractor shall be solely responsible to maintain accurate records as required by the prevailing wage statute and to obtain and furnish all such certified records to the Illinois Department of Labor as and when required by Statute. The Contractor agrees that, prior to making any payments to its own laborers, workers or mechanics or to any subcontractor that it will determine and pay the then-current prevailing rate of wage as determined by the Illinois Department of Labor and posted electronically The City of Moline reserves the right to request the Vendor to file a statement that no City of Moline official or employee has an interest in the proposal for purchase.
