It is the intention of this request for proposal to provide general requirements related to the provision of these specific services. All sections of this request for proposal except Sections B, C, D, and E below will serve as an integral part of the pricing agreement/contract upon the Authority's acceptance of any successful Provider's offer. Providers shall submit two (2) complete copies of their proposals for review. Proposals shall be submitted on the official request for proposal forms only. Providers are required to return only the official request for proposal forms from this request packet, properly completed and signed by an authorized representative of the company in addition to an original Certificate of Insurance as described in Section P. Providers must also submit for consideration any manufacturer's literature, specifications, or other pertinent information with the official request for proposal forms. The words "MANHOLE REHABILITATION SERVICES" must appear on the exterior of the envelope or package in bold letters. No responsibility will be attached to any employee of the Authority for the premature opening of a proposal that is not properly addressed and identified. All proposals will be opened and pricing (only) read aloud at that time, but a final decision may not be made for several days until all information on the official request for proposal forms and any other information supplied can be examined in detail. A tabulation of the total cost per proposal submitted will be made available to those persons attending the opening. The Authority specifically reserves the right to accept or reject any or all proposals and to make any such decision as would be in the best interests of the Authority.
