The Fox River Valley Public Library is issuing this Request for Proposals (RFP) to solicit proposals from qualified, licensed vendors who specialize in plumbing services to furnish all professional services, equipment, labor and materials necessary to provide for the following that is listed at the Fox River Valley Public Library District located at 555 Barrington Ave., East Dundee, IL Scope of work Provide total cost for service and supplies for the following: Lower Level Staff Restrooms Replace 2 sensor lavatory faucets (supplied by library) Install sloan sensor valves for (4) four toilets Provide new toilet seats for (4) four toilets Lower Level Public Restroom(s) Replace mens toilet in handicap stall with proper ADA toilet Install sloan sensor valves for (4) four toilets Provide new toilet seats for (4) four toilets Replace (4) four restroom sink auto sensor faucets Replace (4) four restroom sink auto sensor soap dispensers Kitchen Replace kitchen sink faucet with sensor kitchen faucet Main level restroom(s) Replace womens toilet in handicap stall with proper ADA toilet Install sloan sensor valves for (4) four toilets Provide new toilet seats for (4) four toilets Replace (3) three restroom sink auto sensor faucets Replace (3) three restroom sink auto sensor soap dispensers Note: After evaluation, may need to replace defective mixing valves where needed The vendor/contractor shall possess and maintain Workers Compensation Insurance, General liability Insurance and Contractual Insurance as set forth and provide copies of certificates of coverage
