Renovation of a mixed-use development in Sykesville, Maryland. Completed plans call for the renovation of a medical facility; and office development.
Contract 1. The contract resulting from this solicitation shall be a fixed price contract. 2. The Department reserves the right to reduce or withhold contract payment in the event the Contractor does not provide the Department with all required deliverables within the time frame specified in the contract or in the event that the Contractor otherwise materially breaches the terms and conditions of the contract. At the scheduled site visit, each bidder will be permitted to examine the campus and applicable building, familiarize themselves with the full nature and extent of the work and obtain answers to questions about or clarification of the contract. It is the sole responsibility of the Contractor to become familiar with the campus, applicable building, and the contents of these specifications. Failure to do so does not relieve the successful bidder from their obligations to comply with all aspects of this bid package for the amount specified in the bid. C. Vendor Experience and References a. The vendor MUST list three references in the transmittal letter. All references shall include the name address of the reference company, hospital or organization, a brief description of the type, size, and scope of services being provided and indicate the name, title, and current telephone number and email address of a specific contact person. D. Transmittal Letter A transmittal letter prepared on the Bidder's business stationary MUST be uploaded on eMMA with the bid documents. The transmittal letter should be brief, but must include the following: a. Three references as described above b. Color options as reference in Section III.B.d. and III.B.h. below c. "Attachment C. Bid/Proposal Affidavit" d. "Attachment H. Conflict of Interest Affidavit and Disclosure" The letter shall be signed by an individual or corporate officer who is authorized to bind his firm to all statements, including services, material availability, timeliness and prices contained in the bid. E. Selection Process The contract resulting from this solicitation shall be awarded to the responsible and responsive bidder whose bid meets the requirements noted in this solicitation document, and is the most favorable bid price (lowest cost). III. TECHNICAL SPECIFICATIONS A. SCOPE Removal of remove the existing carpet, cove base and thresholds and installation of brand new carpet tiles, cove base and thresholds in twenty-four (24) offices in the Administration Building on Springfield Hospital Center's campus. DESCRIPTION OF WORK a. The contractor shall remove the existing carpet, cove base and thresholds from the following rooms: 100,101,102,103,104,105,108,108A,111,111A,111B,112,113,113A,116,116A,117, 120,120A,120B,120C,123,123A,123B. b. The contractor shall install new carpet tiles, cove base and thresholds in the following rooms: 100,101,102,103,104,105,108,108A,111,111A,111B,112,113,113A,116,116A,117, 120,120A,120B,120C,123,123A,123B. c. The contractor shall provide and install to the manufacturer's specifications Patcraft Homeroom 26 Carpet tiles. All submitted bids are for the specified carpet brand and type unless an approved equal is specified by Contractor and approved by the facility through the Procurement Officer. d. Contractor shall submit carpet color options available within the bid price with transmittal letter/response on eMMA. Color will be chosen by the Contract Monitor after award of contract. e. Any product substitutions following award must be approved by the facility's Contract Monitor. f. The Contractor shall install carpet to ensure that finished installation consists of a 100% firmly bonded to the surface, showing no seams or grain changes, free of ripples, lumps, loose threads, raw edges, neatly bound and tightly fitted edges. 4 g. Thresholds at every transition to tile are required to be installed by Contractor. h. Contractor shall submit threshold color options available within the bid price with transmittal letter/response on eMMA. Color will be chosen by the Contract Monitor after award of contract. i. Carpet tile should be installed with a manufacturer approved adhesive. j. Contractor shall provide all measures in the performance of this project to ensure the safety of all occupants, visitors, staff and workmen in the project areas. k. Contractor shall repair, at no additional cost to the owner, any areas damaged during this project to the reasonable satisfaction of the facility. l. Contractor shall dispose of all debris resulting in removing/replacing the carpet. All debris shall be disposed of in accordance with all Federal, State and local regulations. m. Scheduling and execution of the work shall be done at the convenience of the facility. Work can be completed Monday through Friday between the hours of 7:00AM and 3:30PM. No work shall be permitted on State of Maryland recognized holidays or weekends without prior approval of the Hospital representative. n. Within two (2) business days of notification, apparent awardee must provide for all persons coming to campus in relation to the contract resulting from this solicitation proof of COVID-19 vaccination or documentation of a medical or religious exemption approved by Maryland Department of Health's (MDH) Office of Equal Opportunity Programs (OEOP).
