CR Number: 2121299 Scope of Work: Stony Brook Medicine consists of the Stony Brook Hospital (Hospital), Linear Accelerator (LINAC), Heart Institute (HI), and the Major Modernization Project (MMP). There are a total of 24 existing ATSs located throughout Stony Brook Medicine that needs to be evaluated for replacement. An Engineer (Consultant) shall provide all professional services as may be necessary for the replacement/upgrade and installation of the hospital's ATSs. These services shall include, but not be limited to, the following: Survey and collect field information. Provide a new updated One-line diagram of all Stony Brook Hospital (Hospital), Linear Accelerator (LINAC), Heart Institute (HI) and the Major Modernization Project (MMP). The one-line diagram is to include, but is not limited to, Substations - Normal/ Emergency-ATS (44) - Distribution Panels - Branch Panels - affected areas within the hospital. Service-Disabled Veteran-Owned Set Aside: No Minority Owned Sub-Contracting Goal: 23% Women Owned Sub-Contracting Goal: 7% Service-Disabled Veteran-Owned Business Contracting Goal: 6% If the bidder cannot make the required certification, such bidder shall so state and shall furnish with the bid a signed statement that sets forth in detail the reasons that the bidder cannot make the certification. After review and consideration of such statement, SUNY may reject the bid or may decide that there are sufficient reasons to accept the bid without such certification. The certification required above can be found on Exhibits E and H which the bidder must submit with this bid. Contract term: 4 years
