The Sherwood Cass R-VIII School District ("District" or "Owner") is seeking qualified vendors to provide all labor, materials, equipment, and accessories to install inclusive playground equipment. Proposals must be submitted in accordance with the requirements as specified in this RFP to be deemed responsible and recommended for contract award. The evaluation of proposals will be made based upon the cost of the project, references, and equipment options available at the various pricing levels. In accordance with Missouri Revised Statute Section 177.086 and 177.088, the Sherwood Cass R-VIII School District is accepting sealed proposals for the inclusive playground equipment installation and purchase. Please see the scope of work for the exact areas for the proposal. For questions or to view the site any time prior to the submission deadline please contact: Dr. Steve Ritter Superintendent 660-499-2239 ext. 400 steve.ritter@sherwoodk12.net All Proposals must be good for forty-five (45) days from the proposal due date. The work includes, at a minimum, leveling of landscape, installation of playground surface and equipment, inspection of all surfaces and equipment for safety, and other work related to installation. Proposals should include: Mock-up proposals of $100,000, $150,000, and $200,000 worth of playground surface and equipment. The surface area to be installed will be a turf cushioned surface. Preference is that all equipment be designed for a variety of students' needs. Key Information: Vendors are responsible for measurements. Please make an appointment w/ Dr. Ritter or Mr. Jeffries (Head of Facilities). Any additional information will be listed as an addendum to this document and will be noted on the district's RFP website (please check this regularly). No updates will be posted after April 21, 2023. No work is to be done on any other existing playground areas. Proposals should include recommendations regarding maintenance All damaged and/or discarded materials are to be removed from the grounds by the vendor. Additional Information on Scope of the Work Vendor Responsibility: The successful vendor will be required to provide the District at the time of contract award, a certificate of insurance indicating that the contractor has, and will keep in effect for the term of the contract, the following types of insurance: Worker's Compensation: State: Missouri Statutory Employer's Liability: $100,000 per accident $500,000 Disease, policy limit $100,000 Disease, each employee Bodily injury: $1,000,000 combined single limit Property damage: $1,000,000 combined single limit Products and completed operations to be maintained for one (1) year after final payment: $1,000,000 combined single limit Property damage liability insurance shall provide X, C and U coverage. Broad form property damage coverage shall include completed operations. Contractual Liability: Bodily injury: $1,000,000 combined single limit Property damage: $1,000,000 combined single limit Personal injury, with employment exclusion deleted: $1,000,000 combined single limit Business auto liability (including owned, non-owned and hired vehicles): Bodily injury: $1,000,000 combined single limit Property damage: $1,000,000 combined single limit If the General Liability coverages are provided by a commercial liability policy, the: General aggregate shall be not less than $5,000,000. This shall be confirmed by a Certificate of Insurance. Fire damage limit shall be not less than $50,000 on any one fire Medical expense limit shall be not less than $5,000 on any one person. Sherwood Cass R-VIII School District to be named as "Additional Insured" It is the responsibility of the Vendor to meet all prevailing wage requirements and to maintain compliance. All penalties to be paid by the vendor if prevailing wage requirements on this project are not met. The Vendor shall be responsible for all damages which may occur as a result of any breach of or damage to the building during the remodel and restoration process, and shall be required to remedy same and to obtain and maintain insurance adequate to the requirements of the Sherwood Cass R-VIII School District in respect of same. Submit proposed scheduling for the submission and/or completion of the work to be performed. In responding to this RFP, a Vendor shall be deemed to have accepted and agreed that the following conditions are acceptable to it and that it shall be responsible for the following matters and any associated costs: All necessary designs and related calculations. Coordinating all work with appropriate Sherwood Cass R-VIII School District's Facilities Department staff. Providing Sherwood Cass R-VIII School District with copies of all related design specifications and drawings. Vendor shall be responsible for all costs and the performance of all tasks required for the installation. Vendors submitting proposals will include technical information on the materials used including but not limited to the surface materials and equipment materials. Sherwood Cass R-VIII School District Responsibilities: Following Sherwood Cass R-VIII School District's acceptance of a Vendor's proposal (and, depending on the applicable timing parameters and assuming the parties have reached agreement to proceed with negotiations concerning the remodel and restoration), Sherwood Cass R-VIII School District's involvement will include: Providing access to the site for the installation to occur Assigning Facilities Service Staff to consult with the Vendor during the installation period Providing space to house the Vendor's equipment. The successful proposals of the work will be required to furnish performance and payment bonds in an amount equal to one hundred percent (100%) of the contract price, giving as security an approved surety bond company bond, which shall meet a Class A rating by the BEST'S INSURANCE RATING GUIDE. The performance bond shall cover both labor and materials.
