Renovation of a fire / police facility in Toledo, Ohio. Completed plans call for the renovation of a fire / police facility.

Toledo Department of Fire and Rescue is soliciting bids for the replacement of Fire Stations #5, 14, and 16's roofs according to relavtive RFQ. Ensure said bids factor in a total project completion time of 90 days from date of Purchase Order issue. In order to be responsive, all bids must have the following with submittal: Project Paperwork Line Items - will auto-populate with e-bid Bid Guaranty (bid bond or certified check / cashiers check on a solvent bank) Signed Contract by the Vendor Child Support Affidavit Bureau of Workers Compensation Certificate Tax Exemption Certificate Insurance Certificate, naming the City of Toledo as an additional insurer If using a check as the bid guaranty, the check must be scanned and uploaded with your bid. In addition, the check must be received by the Division of Purchases and Supplies no later than the due date and time to be deemed responsive. No Company / No Personal checks will be accepted. Checks shall be made payable to City of Toledo. If your company does not provide a bid guaranty, your bid submittal will be deemed non-responsive. (TMC 187.09) The following items will be required upon awarding the contract: Performance Bond and PLA, if applicable. Prevailing Wages are required for this project. It is the obligation of a contractor or subcontractor to make payment in accordance with the prevailing wage determinations. City forms must be filled out and attached to the bid: Project Paperwork (Acknowledgment and Acceptance of Conditions Contract Specifications; Toledo Municipal Income Tax Compliance; Public Utilities Compliance; MBE Good Faith and Goal Commitment; EEO Form; Primary and Subcontractor Forms and Debarment and Suspension Certificate; Best Bid Criteria); Signed Contract by Vendor CEO/President; Current Workers Compensation Certificate; Insurance Certificate listing the city of Toledo as an additional insurer; Child Support Affidavit; and Tax Exemption Certificate. Question Deadline 05/25/2023 at 4:00 PM ET Type of Award: Lump Sum Bids for this project can be accepted electronically. Bids prices shall be to two (2) decimal places (i.e. $0.00). Only one (1) price per line item will be accepted with this bid. Bid Bond 5.00% Payment Bond 0.00% Performance Bond 100.00% Liquidated Damages:1% or $250 per day. Project Duration:90 days.
