Removal & Disposal of Property Contents: 1. Removal and disposal of all personal and household items located at properties (including appliances, tires, food, furniture and personal items left behind in the house, garage and yard). The interior of the houses, garages and yard are to be completely free of all non-building items upon completion. The end result is to have a totally clean house/garage and a debris free yard. Other items may be required to be removed by the Demolition Manager to allow the asbestos abatement contractor to access structural components of a property containing asbestos. 2. Vehicles, hot water tanks, plumbing, furnaces, etc. are not to be removed as part of a clean-out, except with express written permission from the Demolition Manager. Waste Disposal : 1. All clean-out related waste must be sent to a pre-approved certified recycling facility and/or EPA approved licensed landfill a. DUMP TICKETS: The City will require copies of all dump tickets to be included with the invoice, by property address. Invoices will not be paid until all dump tickets have been submitted. b. Separate dump tickets are required for tire disposal. 2. The clean-out contractor must list the dump sites they will be using on the RFQ form. The City of Shaker Heights is seeking qualified contractors to perform clean-outs of residential and/or commercial and/or fire-damaged properties as needed by the City, including emergency situations. The City intends to enter into one-year requirements contracts with one or more qualified contactors who are able to provide these services, with an option for the City to extend the contracts for two addition one year periods. Once the contracts are in place, the City will request quotes from the selected contractors for each clean-out project as it arises and award each clean-out or group of clean-outs to the lowest and best proposing firm. Bid Description : 1. Proposing firms must submit one (1) original qualifications by the deadline stated above. 2. The proposal must include the completed Qualifications Form. 3. Provide at least three (3) references. Include current contact names and telephone numbers. 4. Qualifications must be submitted by the deadline stated above by email only. Faxed or mailed applications will not be accepted. Proposals submitted after the deadline will not be considered. 5. All submittals must be labeled "Clean Out Services" The City will not guarantee a minimum number of clean-outs during the term of the contract. Asbestos surveys, asbestos abatements, and demolitions are provided through separate contracts.
