1. Conduct an overall system assessment to evaluate whether the fall protection system is appropriate to address the fall hazards that exist. This would also include an evaluation of fall clearance distances. 2. Interview workers that use the system to understand the procedures and related work practices. 3. Perform a qualified person inspection of the structure associated with each anchorage point, in accordance with standard engineering practice. This inspection will evaluate the condition of the structural support for the fall protection system. 4. Perform a qualified person evaluation of the structural support for the fall protection system, in accordance with standard engineering practice. This includes analysis of the existing structure and anchors to determine capacity to support the fall arrest system. 5. Perform a competent person equipment inspection of each component associated with the fall protection system including: a. General Inspection of the cables b. Overhead intermediate bracket spacing and installation c. Cable tensioners d. Shock absorbers e. Hardware components not properly installed or used f. Signs of deterioration on the structural steel and equipment components included in the inspection g. Signs of tampering with the components of the fall protection system, such as partial or complete disassembly h. Indications of the system being impacted by a fall incident or other forces that might require the system to be taken out of service until repaired or replaced 6. Load testing of the by a qualified engineer per ANSI Z359 recommendations. 7. Review the applicable fall protection use and rescue procedures to determine if any modifications are required. 8. Review the training provided to the authorized and competent persons associated with these fall protection systems. Evaluate the training against guidelines provided by OSHA and ANSI. 9. Meet with Water Supply and Treatment representatives to review the findings outlined in our report. 10. Compile the certification record. The certification record will serve as a consolidated document that describes the installed systems' design, construction, and proper use Local and certified MBE, WBE or SBE 10% or 2. Local and not certified MBE, WBE or SBE 5% or 3.
