Site work for a mixed-use development in Hazard, Kentucky. Completed plans call for site work for a residential development; and water / sewer project.

Project Description: In Total, the Construction Limits Will Consist of Approximately Three and a Half (3.5) Acres. One (1) Acre Within the Slide, One (1) Acre Within the Waste Area, One (1) Acre Within the Access Road and Ditch Area and a Half (0.5) an Acre, Which Will Incorporate Disturbed Areas Around the House, Garage and Driveway. All Other Seeding Will Utilize the Aml General Reclamation Mixture Found in the Standard Specifications on Page a-6. Revegetation Activities Shall Be Completed Within the Recommended Seeding Window Found on Page a-5. Any Seeding Activities Outside of This Window Must Be Approved by the Branch Manager. Site Preparation Will Be Utilized on Three (3) Acres, Which Will Include One (1) Acres of Slide and Two (2) Acres of Waste Area, Access Road and Ditch Excavation Area. The Area Around the Home Will Not Require Any Site Prep. Excavator Hours Can Be Used to Explore the Slope and Transition Area to Determine Rock Depth Prior to Slide Excavation. The Project Includes One (1) Waste Area Approximately One (1) Acre in Size and Located Along an Existing Mine Bench, Above the Home. An Existing Dirt Road Runs Parallel to the Base of the Highwall. Some Trees Have Grown Within the Road and May Need to Be Removed, but Most Are Small in Diameter. Trees and Brush Will Be Windrowed Off to the Side. A Subdrain Will Be Installed at the Base of the Highwall to Alleviate Existing Drainage Issues and Daylight Into an Adjacent Natural Drain Where a 4' F.b. Class Iii Ditch Will Be Installed. Excavated Material Will Be Placed Against the Highwall and Graded at a 2:1 Slope. The Existing Access Road Will Be Left in Place for the Landowner but May Need to Be Moved Accordingly, After Waste Material is Backfilled. An Ecb Diversion Ditch Will Be Installed at the Top of the Transition Zone and Along The Disturbed Access Road. It Will Daylight Into the Installed Class Iii Ditch at Whatever Elevation Is Determined by the Onsite Engineer During Construction. Location May Vary Depending on Final Grade. Silt Bales Will Be Installed at Multiple Locations Within the Work Limits. Silt Fence Will Be Installed Between the Strip-to-rock Area and the Mobile Home. A Temporary Type B Silt Trap Will Be Installed at the Furthermost End of the Installed Class Iii Ditch to Capture Sediment During Construction. Upon Project Completion, the Silt Trap Will Be Removed and Used to Armor the Ditch Outlet. Mobilization is Setup to Provide for Construction Equipment to Be Delivered to the Site. Slide Abatement Includes Stripping the Existing Unstable Slope to Rock. A 30' Transition Zone Will Be Included Around the Perimeter of the Slide Area to Blend the Stripped Area and Vegetated Area Together to Minimize Future Erosion. Side Slopes Will Have a 3:1 Maximum Slope and Will Be Covered With Ecb After Revegetation is Complete. Silt Fence at Base of the Strip to Rock Area Shall Be Left in Place Until Vegetation Germinates to Prevent Sediment From Filling Up Concrete Blocktied Mat Ditch Below. No Geotechnical Drilling Was Performed for This Site, So Rock Depths Are Unknown for Soil and Rock Depths Within the Slide Area. Earthwork Work Bid Item is Included to Strip Slide Down to Rock and Transport Material Upslope to The Designated Waste Area. It Also Includes All Ditch Excavation and Hauling of Ditch Material. All Bids Shall Be Accompanied by a Bid Guarantee (in the Form of a Bid Bond or Certified Check) of Not Less Than Five (5%) Percent of the Amount of the Total Bid. A 100% Performance Bond and a Separate 100% Payment Bond Shall Be Furnished by the Successful Bidder. All Bonding and Insurance Requirements Are Contained in the Instructions to Bidders and/or General Conditions. Bonds Should Be Executed by a Surety Company Authorized to Do Business in the Commonwealth of Kentucky. The project consists of a shallow, medium-sized slide, extensive mud and drainage around and under home, threat of large trees falling towards home from slide area and drainage issues on the existing mining bench above. The project is located in Perry County, within the Krypton Quad and PU #0402. Directions to the site are as follows: From the junction of KY-80 /Morton Blvd (Shell Gas Station), continue to travel past Morton Blvd and take Exit 59 to KY-15 N ramp to Jackson. Turn left at light onto KY-15 N and travel 5.1 miles to Jefferson Heights/El Dorado Lane on right. Top of slide can be accessed from El Dorado Lane at coordinates 3720'28.5238"N, 8315'53.9725"W. Hignite residence and toe of slide can be accessed from Jefferson Heights Road at coordinates 3720'23.1988"N, 8315'52.0534"W. The Hignite residence is located at 114 Jefferson Heights Road, Hazard Ky 41701. Mr. Hignite can be reached at (606) 436-5531 on his cell. In total, the construction limits will consist of approximately three and a half (3.5) acres. A temporary type B silt trap will be installed at the furthermost end of the installed Class III ditch to capture sediment during construction. Upon project completion, the silt trap will be removed and used to armor the ditch outlet. Mobilization is setup to provide for construction equipment to be delivered to the site. Slide abatement includes stripping the existing unstable slope to rock. A 30' transition zone will be included around the perimeter of the slide area to blend the stripped area and vegetated area together to minimize future erosion. Side slopes will have a 3:1 maximum slope and will be covered with ECB after revegetation is complete. Silt fence at base of the strip to rock area shall be left in place until vegetation germinates to prevent sediment from filling up concrete blocktied mat ditch below. No geotechnical drilling was performed for this site, so rock depths are unknown for soil and rock depths within the slide area. Earthwork work bid item is included to strip slide down to rock and transport material upslope to the designated waste area. It also includes all ditch excavation and hauling of ditch material to the designated waste area. Earthwork bid item is also for grading the access road leading up to the bench, as well as the road located on the bench that will be re-installed after reclamation is complete. Armored HDPE Pipes are included to properly carry accumulated bench drainage, subsurface drainage and surface runoff to the natural drain, which will enhance long-term erosion control and stability of the site. Hoe ram bid item is included to address/reduce the size any boulders too large to move with an excavator during the installation of subdrains, pipes and ditches. Unstable rocks encountered within the slide area will also be hoe rammed and removed from the slope to protect the residence. Debris barrier wall bid item will be utilized directly behind the home and will consist of an 80' long temporary debris wall to protect the home during reclamation activities. Wall should be installed per AML specifications and bales can be used to revegetate the residential area with upon project completion. Utility Relocation, Structure Removal/Replacement and Landscape Allowance are setup to address these features that may require location / replacement to facilitate normal reclamation activities. Site Prep is included, at both the site and waste area to prep the areas for reclamation and material storage. Seeding, Seedbed Prep, Fertilizer, Mulch-Straw & Hay, General AML Seeding and Residential Seeding are included for all revegetation efforts within the site and waste area. Dense Grade and Roadway Stone are included to provide for access during normal reclamation. Bituminous Repair is included to repair "in kind" all asphalt surfaces damage due to normal reclamation activities. Permits will be obtained prior to construction. Multiple trees (equal to or greater than 5-inch breast height) will be disturbed outside of the winter clearing season (October 15 - March 31th). Therefore, bat in-lieu fees will be paid for a total of two (2) acres, which will incorporate the designated waste areas as well as any trees that may need to be taken down along the access road leading up to the mining bench and within the ditch excavation areas. All other trees are located within the designated "slide area". All trees will be hauled to the waste areas and windrowed off to the side. No trees or brush will be combined with any excavated material.
