Remodeling and renovation of a medical facility in Lakeview, Oregon. Completed plans call for the remodel of a one-story above grade medical facility.

Project consists of the remodel and mechanical improvements for the pharmacy clean rooms at the Lake District Hospital. Bidders must be pre-qualified. Bidders may submit by mail or deliver the Contractor's Pre-Qualification Form, which is included in the contract documents, or the State of Oregon Department of Administrative Services Pre-Qualification Application Form on or before February 7, 2024. This is a public works contract and no bid will be received or considered by the public contracting agency unless the bid contains a statement by the bidder, as part of their/his/her bid, stating the provisions of ORS 279.350 (Prevailing Wage Rates) are to be complied with. A copy of the Prevailing Wage Rate Statement is included in the contract documents. No bids will be considered unless fully completed in the manner provided upon the bid form provided by the owner and accompanied by the bid bond or a certified check executed in favor of the owner in an amount not less than 5% of the total amount of the bid. Bid bond or check will be forfeited should the selected bidder neglect or refuse to enter into contract and provide suitable bond for faithful performance of the work. The successful bidder shall provide a performance bond, as hereinafter stipulated, for the full amount of the contract within 10 days after awarding of the contract, i.e. contract date.
