North Broward Hospital District d/b/a Broward Health (Broward Health) is seeking quotations in response to the above-entitled RFQ. The RFQ is attached and contains detailed submission instructions. All required forms must be completed in their entirety and executed by an authorized representative To the extent that there are any questions regarding this RFQ, such questions should be addressed in writing via email to the Bids Department at bids@browardhealth.org No other Broward Health commissioner, officer, employee, or agent should be contacted regarding this RFQ. Any information that amends any portion of this RFQ, which is received by any method other than an Addendum issued to the RFQ, is not binding to Broward Health. Broward Health understands that the supplies, products, equipment, software or services requested in this RFQ may vary from company to company in technique and material. All specifications set forth is this RFQ are to be considered and construed as a general description of function, purpose, and performance of the items desired. Any use of brand names or catalog numbers in the specifications is intended only as a description of the type of product and does not restrict bidding to any endorsed product. No RFQ Response will be disqualified from consideration where items offered by the Bidder are substantially equivalent in quality, purpose and standards, even though it does not correspond exactly to the description contained in the specifications. Where differences exist, they shall be separately identified in an addendum to the RFQ Response with a specific and concise explanation of what differences exist and why such differences do not substantially deviate from the quality, purpose and standards of the items specified. Further data on such differences shall be provided if requested. The items and sizes shown on specification sheets are estimated requirements. Actual purchases may be more or less than quantities shown on specifications, but only the actual quantities required will be purchased.
