sealed bid proposals will be received for the following improvement: Wenonah Avenue Basin Flood Relief Improvements This project consists of installing a 42" diameter storm sewer; jacking 60" diameter sewer casing pipe; replacing curb and gutter, driveway, and sidewalk; patching asphalt pavement; landscape restoration; and other related work. Proposals must be submitted on the forms provided. All bid proposals offered must be accompanied by a bid bond, cashier's check, or certified check in an amount which shall not be less than Ten Percent (10%) of the total amount of the bid made payable to the Order of the President and Board of Trustees of the Village of Forest View, as a guarantee that if the bid proposal is accepted, a contract will be entered into and the performance of the contract properly secured. No bid proposal shall be considered unless accompanied by such bid bond or check. No bid shall be withdrawn after the opening of the Proposals without the consent of the President and Board of Trustees of the Village of Forest View for a period forty-five (45) days after the scheduled time of bid opening. The Contractor and Subcontractor shall comply with all regulations issued pursuant to Prevailing Wage Act (820 ILCS 130), and other applicable Federal Laws and regulations pertaining to labor standards. All bidders wishing to obtain bidding documents must be approved by the Village prior to obtaining bidding documents. All bidders must provide proof that they are prequalified with the Illinois Department of Transportation to perform at least 50% of the value of the work before being issued bidding documents. The successful bidder for the work will be required to furnish sufficient insurance to the Village of Forest View and the Engineer against all damage claims that might arise due to the negligence of the Contractor, and will be required to furnish a Performance Bond and a Labor and Material Payment Bond in the amount of the contract. The contractor shall be paid in cash for the work to be performed. The Village of Forest View reserves the right in receiving these bids to waive technicalities and reject any or all bids. The bidder is specifically advised that the Village is completing the project in partnership with the Metropolitan Water Reclamation District of Greater Chicago (MWRDGC) pursuant to an agreement entered into and between the MWRDGC and the Village. Each bidder is required to comply with the MWRDGC's Affirmative Action Requirements and affirmative Action Ordinance. The successful bidder will be required to include the following sub-contracts as part of their bid: 1. A minimum of $270,000.00 in subcontracts to Minority-Owned Business Enterprises (MBE). 2. A minimum of $135,000.00 in subcontracts to Women-Owned Business Enterprises (WBE). 3. A minimum of $40,500.00 in subcontracts to Veteran-Owned Business Enterprises (VBE). The DBE requirements listed within these Contracts are requirements, not goals. The Village is using a combination of MWRDGC and local funding for the completion of the project.
